The post holder will be able to provide clinical expertise, operational management, to the Virtual Wards (Hospital @Home) Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.
The purpose of the role is to work across the system in a variety of city-wide settings but not exclusive to the community and inpatient units, and to in-reach into secondary care, to identify and assess patients, to prevent avoidable hospital admissions and facilitate early supportive discharge.
The work will be undertaken collaboratively with Secondary and Primary Care, and Community Services, including Urgent Care Response, Early Intervention Team (EICT), and Specialists Services.
The post holder will work attend integrated multidisciplinary teams (MDT’s) to provide advice and support and agreed data to support the Virtual Ward (Hospital @Home) Model.
The post holder will under supervision: Provide aspects of direct clinical patient care, provide expert clinical support and advice and is able to manage aspects of the service within teams which are fundamental parts of this role. Assess, diagnose, and manage a highly specialist adult patient caseload in a variety of settings, which will include assessment, diagnosis, treatment planning therapy programmes and discharge of patients.

You will be joining a community team in the midst of one of the largest Health and Social Care transformations in the country, with exciting changes and developments, preventing avoidable hospital admissions, premature admission to long-term residential care and facilitating early hospital discharge, plus the added bonus of Birmingham being a fantastic place to work and live.

Supporting people of Birmingham and ensuring they are treated in the right place at the right time, evidenced by clinical and performance outcomes. Working multi-professionally, completing assessments and interventions, whilst ensuring the delivery of high quality intervention to patients within their own homes and other community settings.

Detailed job description and main responsibilities

Facilitate teaching and supervision for less experienced members of staff. |
Promote health by collaborative working with patients and their relatives. Provide support to the multidisciplinary team to enable patients to make better choices for their health and that of their families, undertaking health promotion and contributing to the prevention of adverse effects to health and wellbeing. |
Act as a role model to and facilitate others to respect the dignity, wishes and beliefs of patients: involving them in shared decision making and obtaining consent prior to all interventions. |
Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately. |
Contribute to the review and development of integrated care pathways and clinical guidelines to ensure a standardised approach to care |
Participate in the further development of an integrated, multi-disciplinary EICT and Virtual Ward working alongside colleagues in Integrated MDT;s and the wider health and social care economy to facilitate achievement of patient outcomes. |
Triage and co-ordinate other junior members of the team and appropriately delegate specific aspects of nursing and therapy care according to the care plan. |
Participate in MDT’s within Secondary Care, In-patient, UCR and Community Services. facilitating the continuity of patient care ensuring patients and relatives are kept informed of treatments, progress, and discharge arrangements. |
Have responsibility for the safe use of highly complex patient monitoring and support equipment within the work area Communicate effectively verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the remit of the Data Protection, ensuring clinical information is conveyed within a timeframe that ensures patient safety. |
Fully utilise digital platforms, patient systems and IT to support patient monitoring and clinical care delivery. Facilitate safe discharge from caseloads to an appropriate community setting.

Qualifications/Training
Essential criteria
- Registered Nurse Level 1 (Adult)/ AHP
- Evidence of up to date post registration education relevant to the role.
- Degree in relevant subject.
Desirable criteria
- Mentoring or teaching qualification.
- Nurse prescriber
Experience
Essential criteria
- Significant experience as a registered health professional including working in the community , frailty, or older adult setting or care
- Experience working as a registered nurse/AHP in the community.
- Significant experience of a multidisciplinary approach to care
Skills/Knowledge
Essential criteria
- Skills to organise team workload and prioritise
- Awareness of legislation affecting the role including equal opportunities law.
- Ability to evidence application of utilising evidence-based practice
- Work autonomously
- Comprehensive assessment skills and knowledge of the needs of the older person including risk management.
- Computer literacy skills
- Knowledge of current legislation, particularly those affecting community services
- Knowledge and application of the NMC code of Conduct and Scope of Professional Practice.
