Band 5 Community staff Nurse working with the Trinity Team within with West locality.
Providing holistic evidence based care to housebound patients
Main duties of the job
The post holder will work as part of the integrated Multidisciplinary team under the supervision of the Clinical Team Manager. He / She will be accountable for the delivery of high quality care to patients within health centers. Working with other members of the IMT to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GP’s, Tissue Viability and vascular clinics. You will support IMT triaging to ensure patients receive the right care at the right time in the right place.

Essential criteria
- Registered Nurse Level 1 (Adult)
- Mentor and assessor qualification or equivalent
Desirable criteria
- Evidence of continued professional development relevant to role
Experience

- Experience of working in a multi-disciplinary team setting
Skills/Knowledge

- Ability to demonstrate sound knowledge and application of up to date evidence based clinical practice.
