We require enthusiastic and motived individuals to work as part of the Single Transfer of Care Team - Central Hub. The role of a Hub Coordinator (Ward based) is fast-paced, busy, with changing priorities, and we are seeking staff to join this highly valued team. Supporting effective patient flow across all Pathways for admission and discharge, your contribution to the work can be both rewarding and influential as the service continues to develop and evolve.
Main duties of the job
For all posts there will be a requirement to actively support the administrative processes for referral management, coordination and support.
If you have a passion for supporting patients, enjoy the challenge of problem-solving and using your initiative on a daily basis, whilst liaising with many different levels of staff; and patients’/carers, then we would like to hear from you.
The right candidate will have a natural interest in being a committed member of a busy team, providing support with the planning and facilitation of admissions and discharges for all patients, and be confident and able to work with minimum supervision at times of pressure. We pride ourselves on high service delivery and patient-centred care. A willingness to be flexible across the service we deliver is required, and you will need to be both proactive and reactive to keep the flow of patients moving through the system.

Qualifications / training
Essential criteria
- GCSE (A-C) level Maths and English (or equivalent)
Desirable criteria
- NVQ Level 3 in Administration
Experience
Essential criteria
- Relavant experience of administrative and clerical tasks
- Experience of face-to-face working with patients and / or the public
- Experience of collating data/information
- Experience of working as part of a multi-disciplinary team.
Desirable criteria
- experience working on a ward
- Relevant experience in health related work
Skills/knowledge
Essential criteria
- Good keyboard skills with an ability to confidently capture and update information
- Good understanding of hospital operational working – patient flow
- Awareness of MDT working within a ward setting
- Good understanding of the need for confidentiality and working within the boundaries of this
- Demonstrate effective and accurate written and verbal communication skills
Desirable criteria
- Awareness of hospital admission and discharge processes

- Ability to convey sensitive information in an empathetic manner to patients, relative/carers and staff as required so that patients’ dignity and respect is maintained at all times

Applicant requirements
