Join East Community Assertive Outreach Team on an exciting Band 6 opportunity, offering to a motivated and innovative NMC registered nurse. While previous experience as a Community Psychiatric Nurse (CPN) is desirable, it's not required.
East AOT covers a diverse catchment area, providing a unique opportunity for professional growth while supporting a culturally rich community. As part of the team, you'll gain insights into the workings of assertive outreach, including pre-admission processes and strategies to prevent admissions.
Working alongside a highly motivated, experienced, and dynamic team, you'll participate in weekly MDT meetings, Wednesday afternoon case discussions, regular management supervision, and clinical team supervision, safeguarding supervision and including Behavioural Family Therapy supervision.
Our approach focuses on supporting adults aged 25 and above, especially those who historically avoided mental health services, using flexible and creative engagement methods.
Main duties of the job
Effective communication, competent risk assessment, and a genuine interest in supporting individuals with severe and enduring mental health issues and their carers are essential. We are looking for someone who shares our values of inclusion, compassion, and commitment. You'll demonstrate a commitment to high-quality care, utilising the care program approach framework, and working autonomously while collaborating effectively within the multidisciplinary team.
This role involves liaising with partner services and stakeholders, emphasising the recovery model and team collaboration. A valid driver's license and willingness to travel across the Birmingham & Solihull area are required.
Shifts include 9-5 and 12-8 on weekdays, with weekend shifts from 9-5. As a care coordinator, you'll manage a caseload of up to 15 patients. Please refer to the attached job description and person specification for further details.

Essential criteria
- Registered Mental Health Nurse (part 3 or 13)
- NMC registration
Desirable criteria
- ENB812 or equivalent
- Supervisory leadership skills
Knowledge/ Experience
Essential criteria
- Significant post registration experience in mental health
- Understanding of the responsibilities under the Mental Health Act
- Experience with SMI
- Effective Communication both written & verbal
Desirable criteria
- Worked in a community setting
