Job overview

An exciting opportunity has arisen for a hub manager for the BSMHFT Community Mental Health Team for Homeless People, Homeless Health Exchange Primary Care Team and the NHSE funded Mental Health Outreach Team for Rough Sleepers.

All teams operate city wide to meet the needs of Homeless adults with enduring mental health problems and often substance misuse issues who require significant engagement due to their chaotic lifestyle. The teams work in partnership with other homeless commissioned statutory and third sector organisations for accommodation, housing and tenancy support, physical health care and substance use services.

The CMHT is a small team whose CPN’s work with a consultant psychiatrist to provide secondary care level mental health interventions with a focus on stabilisation of lifestyle, recovery and moving out of homelessness.

The team working with rough sleepers has an engagement, assessment, referral and signposting case management role to manage mental health issues, in order to allow other agencies interventions to support the individual off the streets and start their journey away from street homelessness into a less chaotic and damaging lifestyle.

 

Main duties of the job

Demonstrate clear management responsibility, through ensuring high standards of care and that the smooth day-to-day running of these highly complex services are maintained to the highest level possible. 

Co-ordinate, manage and supervise the activities of all staff working within these teams to provide a highly visible presence in all clinical areas, leading and supervising practice and acting as a role model for professional issues including non-medical prescribing. 

To ensure effective use of clinical systems – this may entail training staff to input into BSMHFT and Primary Care providers' clinical systems.

To ensure effective process and data is viable to monitor and report on the effectiveness of the services. 

Ensures that the day-to-day care provided within your teams meets the complex needs of the individual service users. 

Provide regular management and clinical supervision to all staff under sphere of responsibility.

Person specification

Essential

Essential criteria
  • Registered professional with up-to-date registration. Masters Degree in relevant subject (or working towards) or equivalent relevant experience. Further professional registration.
  • Significant operational / team management and leadership experience. Experience of budgetary control and management. Experience of community nursing and or multiagency working.
  • Communicates with people in a form and manner that is consistent with their level of understanding, culture, background and preferred means of communication.
Desirable criteria
  • Quality Assurance or Management qualification.
  • Experience of working with homeless people and other vulnerable groups.
