Job overview
The post holder will be required to work as an integral member of a multidisciplinary/multi-agency team offering improved early identification and assessment of people with a possible diagnosis of dementia. This will include culturally appropriate assessment of service users referred by primary care using a range of evidence based assessment tools
focused at a primary care interventions level. The post holder will be required to work as an integral member of a multidisciplinary/multi-agency team offering improved early identification and assessment of people with a possible diagnosis of dementia. This will include culturally appropriate assessment of service users referred by primary care using a range of evidence based assessment tools focused at a primary care interventions level. Assessment clinics will take place in a range of settings and times, so flexibility to work in a clinic setting and within the commnity based on service needs, is essential.
Main duties of the job
To receive, within a single point of access system, referrals via GP for people with a possible or suspected diagnosis of dementia
To undertake a culturally sensitive comprehensive generic assessment and diagnosisof dementia
To deliver pre and post diagnostic counselling, to include disclosure of the outcome of the assessment to the person (and their family if appropriate), including the sharing of diagnosis and its potential implications
To contribute to the maintenance and ongoing development of standards of clinical practice within MAS, participating in service evaluation
To signpost to other services and contribute to a seamless pathway to secondary mental health services where these are indicated
Delivering integrated care in partnership with other local health, social care and voluntary organisations
To deliver a range of therapeutic interventions to groups of people with recent diagnosis of dementia

- Registered RMN or registered OT with degree.
- 6 months post graduation experience in mental health
Desirable criteria
- Experience working with service users with dementia in professional capacity
knowledge of working in a community setting
