What a great time to be joining Team BSMHFT! The BSMHFT RECONNECT Team are seeking an enthusiastic and experienced Recovery and Wellbeing Substance Misuse Practitioner to work alongside the team!

RECONNECT is an established part of the BSMHFT Health & Justice Vulnerability Service supporting individuals aged 18+ who are approaching release from / have recently been released from secure sites (HMP estates and Immigration Removal Centres) returning / relocating to the Birmingham and Solihull area who have an identified health need which could negatively impact on their ability to engage with community services.

RECONNECT take a partnership approach to ensure that individuals leaving prison have their needs met, in sustainable ways, to reduce their risk of reoffending, through providing coordinated intervention, support, and risk management across multiple agencies, including health, criminal justice, local authority, substance misuse services, employment services and voluntary sector organisations.

This exciting role offers the suitable candidate the chance to contribute to the implementation of support, with opportunities to apply clinical expertise and forge new care pathways in the community with a range of non-NHS providers, offering support to a service user group who have traditionally been underserved. 

Main duties of the job

You will work as closely as part of a team to individuals who have been assessed and accepted to the RECONNECT Caseload and will be responsible for contributing to clinical risk assessments and developing and delivering individualised care to support individuals to engage with their communities and address their substance misuse needs, this may include 1:1 working or supporting/facilitating recovery activities/groups within the service cohort. You will also work extensively with a range of partner organisations to ensure that individuals receive coordinated care and act as the predominant point of contact for the team for substance misuse support. 

The successful candidate will have extensive experience of working with substance misuse needs and be willing to work alongside the team to further review and develop this new and exciting pathway. It is expected that you will have experience of partnership working across health, social care, voluntary sector, employment, and criminal justice agencies. Relevant knowledge and experience of working with underserved groups would be an advantage. You will require good organisational, interpersonal and communication skills.

The post will be based at Thomas Telford House (Ardenleigh Hospital) alongside Birmingham Magistrates Court, there is also the expectation for community working and the travel between sites. There will be requirement to support / complete assessments / pre-work within HMP Birmingham.
