The All Age Continuing Health Care (AACHC) team are seeking a motivated, passionate and enthusiastic Registered nurse (RSCN/ SCPHN) with a can-do attitude to join our dynamic team. The opportunity to work autonomously and collaboratively in a multi-agency environment to support the assesment and review processes.
We are looking to appoint a qualified nurse, with experience of working with children and young people (CYP) with complex health needs, to help us deliver our NHS Continuing Care service across Birmingham and Solihull. The role is varied and interesting and involves assessing CYP mainly in community settings, be that their own home, or a specialist setting. The assessment is a crucial stage of the process, when considering if a CYP is eligible for their care to be commissioned and funded by the NHS.
CYP are often referred for assessment due to a significant change in their care needs, resulting in the consideration of NHS Continuing Care being necessary. We need a nurse with the ability to holistically assess care needs, who is compassionate and caring with the skills to support CYP during, what can be, a difficult time.
It is a clinically led and supportive team working with some of the most vulnerable CYP in our community. There is an opportunity to plan care, review its effectiveness, and work with colleagues in the wider health and social care system to deliver the best outcomes for CYP who are found eligible for NHS Continuing Care.
Main duties of the job
- To review any service, and care, commissioned in regards to quality and appropriateness to the CYP need. To ensure that the provider is delivering the requirements of the contract ensuring value for money in all matters relating to packages for NHS continuing care.
- The post holder will work jointly with health, social care and independent sector colleagues to ensure care management processes are appropriate.
- To follow the process laid out in the National Framework for NHS Continuing Care; to include organisation, co-ordination and participation in MDT eligibility recommendations to be verified by ICB.
- Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Care
- The post holder will be responsible for complex clinical caseload comprising of; assessments, undertaking of reviews (3/12 CC review) unscheduled reviews and Fast Tracks and implementation on Personal Health Budgets and ongoing Case Management of patients funded by Continuing Care

Detailed job description and main responsibilities
§ The post holder will be required to provide expert advice and assist in identifying appropriate providers to address the needs of complex individuals.
§ The post holder will be required to attend Multi-Disciplinary Meetings, Acute Care Facilities, individual’s homes and a variety of care provider settings.
§ The role will be required to regularly review individuals in line with The National Framework for NHS Continuing Care, case management requirements.
§ To present comprehensive written reports providing assurance to the ICB that the agreed placement is delivering the care to meet identified needs and it is appropriate to continue commissioning the care plan.
§ They will be responsible for negotiating care costs and following the agreed ICB processes for agreement of care packages.
§ To support the Head of CHC and Lead Nurses in ensuring appropriate individual placements provide safe and high quality care
§ Responsible for Line management of a defined number of Band 6 Clinical Nurse Assessors.
· The post holder will deliver an effective and competent level of service that is consistently person- centred focused for children and young people
· The post holder will liaise with other professionals to facilitate collection of written reports to inform the Continuing Care assessment process. This involves quality assure the completion of holistic and robust continuing care assessments.
· Working in conjunction with the wider ICB and external stakeholders, to implement effective service pathways, ensuring the delivery of key quality outcomes and strategic objectives.
· The post holder will play a key role in delivery of person-centered assessments in order to identify any unmet care need which may result in eligibility for continuing care, ensuring the National Framework for NHS Continuing care for Children and Young People is implemented and complies accordingly with the principles and processes with onward referral to other agencies where appropriate.
· An essential element of the role will include seamless working with other professionals and partner agencies in respect of assessing eligibility for Continuing Care and supporting multidisciplinary teams in the assessment process. This role will ensure that the provision of care achieves individual outcomes and ensures the delivery of efficient quality care packages that is aligned to the Personalisation and SEND agenda and supports the transition of young people into the appropriate adult services.
· The post holder will work in partnership with stakeholders and other professionals across both the public and private sector, education, health and social care partners to ensure a team around the child ethos for Continuing care is met. This would require liaison with the relevant health professionals, education, children with disability social work teams and other wider members of the team around the family.
· Have a very detailed knowledge and understanding of all aspects of continuing care with detailed understanding of the correct use of the national tools (pre-assessment checklist, Decision Support Tools (DST), (Fast Track).
· To facilitate and carry out timely reviews of CYP receiving continuing care and to refer to others as is appropriate.
· To ensure that the service provided is of high quality and is culturally sensitive, promoting enablement of the child/young person and their family and preventing over-dependency on services where appropriate.
· To provide specialist advice and support to the ICB, to facilitate timely and accurate assessment of Continuing Care needs.
· Work with teams and other colleagues across primary, secondary and community care to ensure that all patient care areas meet agreed standards, taking personal action to redress any problems in conjunction with relevant multi-disciplinary teams.
· To work with Continuing Care and Young People Programme Lead, other professionals and partner agencies and provide professional and expert advice and training with regards to the children and young people’s continuing care process.
· The post holder will provide a flexible and accessible service that is responsive to the needs of the child, young people, and their family.
· To support the Continuing Care and Young People Programme Lead in participating in strategic development of the continuing care service and actively contribute to the development of nursing clinical standards, policies and procedures to ensure that implementation of continuing care is sustainable and meets the assessed needs of children and young people.
· To be accountable for the delivery of child specific packages of care, either in the community, residential or school settings designed to meet need and maintain the safety of children and young people known to Childrens Continuing Care.
· To offer assessments and case management to cases requiring or anticipated to require ongoing case management.
· In partnership with other Clinical Managers and Senior Managers to ensure the implementation of the National Framework for Childrens Continuing Care (2016) and continuous service improvement.
· To plan and negotiate with approved private/independent Nursing or Care agencies the implementation of packages of care to enable children with complex health, care needs, and their families to have their nursing and care needs met within the community setting.

- Dual registered sick children's nurse/Registered Nurse with a current NMC registration or equivalent
- In depth experience of undertaking Continuing Care Assessments
- Significant experience of working with children and young people with complex and long-term health needs, disabilities and those with life limited/palliative or end of life
- Good working knowledge of children’s safeguarding .
- Demonstrated capabilities to manage own workload ef-fectively and make informed decisions in the absence of required information, work-ing to tight and often chang-ing timescales to meet agreed actions and out-comes
