Job overview

The All Age Continuing Health Care (AACHC) team are seeking experienced, proactive and  passionate Registered Nurses with current knowledge and recent experience of completing CHC Assessments and CHC Case Management, to join our dynamic, friendly team. This is a rare opportunity to work autonomously and collaboratively in a multi-agency environment, within a fast paced team. The primary focus of the role is Case Management,  supporting individuals and their representatives through all stages of  the  CHC Appeals Process.

 

As a Senior CHC Appeals Nurse you will be responsible for implementing the NHS England CHC Framework for Appeals of CHC funding decisions where patients have not been awarded full CHC funding for their care. This includes the robust review of the previous MDT assessments and decision making processes followed during the assessment process for Continuing Healthcare Eligibility. 

 

 This is an excellent opportunity for an experienced CHC Nurse Practitioner to gain a wide range of experience working with individuals who have a broad range of health care needs and to work in a service that is constantly committed to improving patient care and experience.

Main duties of the job

As a Senior CHC Nurse Assessor in the AACC Appeals Team, you will be

  • using your CHC knowledge and a wide range of your clinical skills to ensure that outcomes are determined in a consistent manner to ensure effective application and interpretation of the National Framework for NHS CHC (2022).
  •  reviewing and progressing eligible applications for review of Periods of Previously Unassessed Care (PUPoC) and
  • ensuring robust eligibility recommendations can be made using the NHS CHC Framework process for completing retrospective Decision Support Tool  assessments.
  • proactively case manage a  caseload comprising of both Current informal and formal Local  Appeal and PUPoC cases, including collating and compiling evidence in detailed review document and maintaining robust, accurate, contemporaneous records within digital databases.  
  • working collaboratively with  all key stakeholders and colleagues within the NHS BSOL AACC Team to review CHC appeals and CHC PUPoC requests to ensure that the NHS Continuing Healthcare Framework and Best Practice Guidance and processes have been adhered to.
  • Providing additional knowledge and expertise, when required, to support  clinical and non-clinical colleagues reviewing issues arising from  both Appeals and Previously Unassessed and Assessed Periods of Care (PUPoC).

Detailed job description and main responsibilities

 The CHC  Appeals Team is an integral part of the wider NHS  BSOL  All Age Continuing Care Team (AACC).  It has strong links with a wide range community, acute and tertiary services and third sector providers.  We also work closely with NHS England and the regional Independent  Review Panels.

This position requires a proactive,  collaborative working style to promote effective relationships within the multi-disciplinary team and with our community colleagues in health and social care is essential.

 The post holder will play a key role in supporting the All Age Continuing Healthcare  Team in delivering effective, responsive services across the BSOL region.; working within the remit of the National Framework for Continuing Healthcare and PUPoC Guidance.

The post holder will have both experience and a high level of  confidence and  understanding/experience of working to the Best Practice Guidance of the NHS Continuing Health Care Framework (2022) 

The post holder will 

  • produce comprehensive reports,  including Care Needs Portrayal documents identifying a patients journey throughout the period of review for both Appeals and PUPoC.
  • have  a high level of communication skills across both written and verbal media,  to be able to lead and  Chair Local Resolution Meetings with patients/individual's and their representatives
  • deliver an effective and competent level of service and consistently deliver a client-focused service.
  •  work autonomously to individual targets and team key performance goals to progress and manage a caseload of patients as directed and
  • support colleagues within the appeals team as required to enable effective case coordination and respond to queries from  all interested parties.

Essential criteria
  • Registered Nurse with current NMC Registration
  • Previous experience of Case Management of Adults with complex healthcare needs
  • Educated to Degree Level or equivalent level of experience within clinical practice
  • Recent experience of completing CHC Assessments
  • Objective evidence of strong verbal and written communication skills
  • Driver with own transport
  • Fully conversant with CHC Framework including Appeals and PUPoC Best Practice Guidance
  • IT literate with experience/confidence in using basic Microsoft Office Programmes
Desirable criteria
  • Relevant experience in working with Multidisciplinary Teams in clinical practice
  • Knowledge and understanding of the role of the AACC Team within the NHS BSOL ICB
  • Evidence of experience in a leadership role within clinical practice
  • Evidence of continued professional development through Nursing Career
  • Fully conversant with CHC Framework including Appeals and PUPoC Best Practice Guidance
  • Observant and concordant with ICB policies for Equality and Diversity
  • Concordant with ICB policies for Confidentiality and Clinical Governance
  • Demonstrates understanding of Quality Assurance and Continuous Improvement in clinical practice
