Job overview

An exciting opportunity has arisen for a highly motivated, enthusiastic District Nurse with a Community Nursing Specialist Qualification to take up the post of Band 7 District Nurse Team Leader within the Community with Core Clinical Services (CCCS)  Division working across the Dudley Borough.

Main duties of the job

The post-holder has overall accountability for the District Nursing Team within their Primary Care Network (PCN), and responsibility for the ongoing assessment,  development, implementation and evaluation of programmes of care for patients on the District Nursing caseload.

Physical Skills

Essential criteria
  • Must be able to commute to meet the demands of the role

Experience

Essential criteria
  • Evidence of leadership / management qualities
  • Evidence of teaching / supervision skills
  • Demonstrates up to date evidence based clinical trends in relation to community nursing
  • Experience at Band 6 in community nursing
Desirable criteria
  • Operational experience in the delivery of disease management/Long term conditions

Knowledge and Training

Essential criteria
  • Up to date knowledge of current Government and Local imperatives and how these translate into local practice
  • Computer literate – ability to use software programmes designed to maximise the contribution to the post
  • Evidence of collaborative working with multi-professional colleagues
  • Knowledge of budget management
  • Knowledge of Governance and Incident Management
Desirable criteria
  • Demonstrable contribution to practice developments in primary care / disease management/long term conditions

Education & Qualifications

Essential criteria
  • First level Registered Nurse
  • First level qualification in District Nursing / specialist Practitioners qualification in District Nursing
  • Evidence of formal Leadership / Managerial Training and Development
  • Evidence of commitment to life-long academic learning
  • Nurse Prescriber
  • Evidence of Continued Professional Development
  • Mentor or practice based learning training
