Job overview

There is an exciting opportunity arisen withing the specialist lung cancer team as our lung cancer care co-ordinator.
The post is for 37.5 hours a week Monday to Friday, over 5 days. 
It is a temporary contact until September 2026.
The successful candidate will be involved in the lung cancer pathway. The successful candidate would be involved with lung cancer patients who have been referred on a Rapid Access (RA) pathway for lung cancer.
Part of being the lung cancer coordinator you will be a point of contact for the following:
  • To liaise and alert the clinical tea​​m if required.
  • Tracking investigations and results.
  • To chase scan appointments, coordinate results, appointments and queries related to these aspects.
  • Signposting to support services.
  • As part of the wider clinical team and MDT to support patients and their families with the elements of a person centred approach to their care.
  • Implement the elements of person centred care.
  • Complete Holistic Needs Assessment and signposting.
  • Treatment summary documentation.
  • Manage our dedicated telephone line.
  • Support the nursing team with emails, referrals, co-ordinating patient care.

Main duties of the job

The purpose of this role is to complement the lung cancer site specific team. To help them support patients through diagnosis, treatment pathways & follow up. To ensure the provision of safe, seamless, appropriate service. This helps maximise the health & quality of life of patient and carers. Improving the quality & efficiency of health care delivery by:

• Being responsible & accountable for their practice and behaviour under the guidance & supervision of a registered practitioner and by working as part of the cancer management team. 
• Working as part of the lung cancer team, following up Holistic Needs Assessment, coordinate the care of the person living with cancers non-complex needs so they can access the right help at the right time. 
• Working with the lung cancer site specific team to provide coordination of high-quality patient care through on-going telephone/ face to face assessment and proactive identification of needs using basic knowledge, approved tools and procedures. 
• Be the single point of contact for all patients on the lung cancer pathway of care. To coordinate care and track, helping manage patient concerns via the helpline and generic email account.
• Navigating the complex health and social care system, both during and following completion of cancer treatment, by building relationships with professionals and facilitating access to these services including generating referrals on the person’s behalf where necessary.

Qualifications and Education

Essential criteria
  • 5 x GCSE’s at level/grade 4 or above including Maths and English or equivalent qualification
  • NVQ level 3 Business Administration or equivalent qualification
Desirable criteria
  • Evidence of continued professional developmen
  • Willingness to undertake learning and development courses

Knowledge & Training

Essential criteria
  • Able to demonstrate advanced use of Microsoft office packages, particularly outlook, excel and word
  • Essential Working knowledge of designing and maintaining excel/access databases
  • Able to meet changing demands and priorities with a flexible approach
  • Competent in the use of information technology systems
