Respiratory Support Practitioner

Job overview

The post holder will form an integral part of the multidisciplinary team approach in providing personalised care interventions to patients diagnosed with respiratory disease and their carers throughout the care treatment pathway. The Respiratory Support Practitioner will work closely with the Respiratory Specialist CNS’s and Physiotherapists to support patients identified to the service by the respiratory outreach teams. They will support with all aspects of service provision, e.g., 
training / equipment management, provide advice and support for the management of patients with respiratory disease. They will also support patients through the virtual ward hospital at home pathway by providing equipment and education to patients to support their discharge under early supported discharge pathway.

Main duties of the job

COPD/Asthma Bundles:
1. Give advice and education to support management of their lung condition, including identifying that patients are on the correct inhaler and their technique is sufficient and selfmanagement plans are completed.
2. Advice on pulmonary rehabilitation and appropriate making referrals.
3. Education in recognising future signs of an exacerbation of their respiratory condition.
4. Offer very brief advice on stop smoking and making referrals to the inpatient Tobacco Cessation Service.
5. Collate and submit data to the National Respiratory Audit Programme (NRAP).
6. Be able to perform ARTP accredited spirometry with reversibility.

Supportive discharges/Respiratory Virtual Ward:
1. Take observations of patients and report back any concerns outside expected parameters.
2. Visit patients in a community setting to perform observations and ensure patient is treated via the correct care pathway.
3. Ordering of Oxygen for patients requiring it for discharge from hospital under the guidance of a prescribing clinician

Person specification

Qualificaitons and Education

Essential criteria
  • NVQ level 3 or equivalent experience
  • Maths and English GCSE grade C or above
  • ARTP Performing spirometry (or willing to complete within 9 months)
  • Evidence of continuing professional development
  • Willing to attend additional training session needed for the role

Knowledge and training

Essential criteria
  • Knowledge of chronic Respiratory conditions
  • Knowledge of patients / clients needs when being treated / care for in a healthcare setting
  • Knowledge of the elements of personalised care initiatives and recovery for patients with respiratory conditions
  • Knowledge of COPD and Asthma care bundles
  • Able to educate patients on their chronic respiratory conditions without supervision
  • Ability to initiate & co-ordinate change to own working practices

Experience

Essential criteria
  • Experience of within in a multi-disciplinary team environment
  • In depth understanding of medical technology appropriate to specialism
  • Be able to deliver training sessions to patients, peers and seniors as required
Desirable criteria
  • Visiting patients in a community setting

Communicaiton and Relationship skills

Essential criteria
  • Able to communicate with patient and educate them on a variety of health promotion incentives
  • Able to recognise and act on appropriate referrals to other services

Analytical and judgement skills

Essential criteria
  • Analytical & Judgement Skills Weighting Ability to use own judgement to resolve problems and queries professionally
  • Understands patient pathways in respiratory
  • Able to manage, interpret and input data onto audit systems independently and accurately
  • Understands own limitations and works within his/her scope of practice

Planning and organisational skills

Essential criteria
  • Ability to cope with distressing situations and deal with conflict
  • Be able to prioritise workload to meet targets and deadlines

Physical skills

Essential criteria
  • Be able to perform patient facing education
  • Appropriate level of physical fitness to undertake manual handling when required
  • Have a full valid driving licence, access to a vehicle and willing to insure for business use
