Job overview

An exciting opportunity has arisen for a triage clinician to join our Neighbourhood Mental Health Team (NMHT) in South Birmingham. The main duties of the role will include triage, assessment and brief interventions for patients with mental health difficulties being referred from primary care.Our community mental health services are well-established teams, as part  of the BSOL community transformation programme, we are looking to increase our Neighbourhood Mental Health Team with this role,  focussing on supporting GP colleagues with initial triage and assessment of mental health difficulties. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Neighbourhood Mental Health Team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.

You will be responsible  for providing  mental health triage, assessment and outcomes based Intervention for service users within the Neighbourhood Mental Health Team.

You will be responsible for triaging new referral’s, identifying need and best course of advice, support or intervention.

You will be responsible for promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.

Main duties of the job

The post holder:

·       Will be responsible for supporting the daily triage and assessment of new referrals

·       Will consolidate historical information for new referrals into the Neighbourhood Mental Health Team, and support the daily triage meeting.

·       Will be responsible for the assessment planning implementation and evaluation of care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.

·       Will be responsible for ensuring effective communication between pathways exist.

·       Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.

·       Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.

·       Support colleagues in the effective assessment and delivery clinical service of the individuals within the assigned case load.
