Job overview

The post holder will assume responsibility for the completion of nursing assessment of patients referred to both Intermediate and Continuing Healthcare and review Free Nursing Care Contribution Eligibility. They will assess patients both in the (Hospital setting) Acute Trust and (Community setting) Primary Care.

Main duties of the job

The post holder will screen and assess referrals to Intermediate Care

and work in partnership with other professionals in the planning,

implementing and evaluation of care.

 

The post holder will work in close collaboration with patients/ carers

actively involving them in decisions to access Intermediate care and in

the implementation and evaluation of their care plan.

 

Telephone Triage patients referred as part of the Hospital Avoidance

scheme and where appropriate, organise appropriate placement or

refer to other Health and Social Care Professionals to ensure patient

safety.

 

Assess patients referred to the Continuing Healthcare Team for Fasts

Track Assessment and organise Care provision, where possible

alongside patient’s PPC, adhering to National Guidance.

 

 Effectively communicate, liaise and work collaboratively with

partners and agencies to ensure a co-ordinated and person centred

service delivery for all patients.

 

Lead regular Multi-Disciplinary Meetings and contribute to admission

and discharge planning and ongoing reviews of patient care needs and

provision whilst in Intermediate Care.

 

Respond to any Safeguarding concerns relating to designated

intermediate care placements as required.

 

The post holder will regularly review their own practice and

professional development through appraisal and personal

development reviews.

Detailed job description and main responsibilities

The post holder will actively contribute to review of current

Intermediate Care Services and actively participate in helping improve

current and new services

 

The post holder will monitor, audit and review care provision and

quality standards with aim to ensure best practise and safe delivery of

care in all Intermediate Care Facilities

 

The post holder will have an understanding of use of various IT

systems and software

 

The post holder will ensure attendance at mandatory training and 

keeping up dated in respect of Knowledge and Skills

 

The Post Holder in the absence of Lead/Deputy will support by

attending hospital delays meetings (Rag Call Duty).

 

Clinical calls to Pathway 3 Placements. 

Person specification

Qualifications

Essential criteria
  • Registered General or Mental Health Nurse.
  • Highly skilled nurse competent to practice within the extended role
Desirable criteria
  • Evidence of ongoing basic qualifications in areas identified
  • Knowledge and ability in Telephone Triage
  • Educated to degree Level /or Equivalent (or working towards this)

Knowledge

Essential criteria
  • Minimum of 2 years qualified experience
  • Proven ability and experience working across organisational boundaries
Desirable criteria
  • Operational experience in an Intermediate Care Setting
  • Demonstrates knowledge and understanding of the National Framework for Continuing Healthcare

Skills

Essential criteria
  • Excellent verbal, written and presentation skills.
  • Knowledge of IT and relevant soft-ware programmes to maximise contribution to the post
  • Demonstrates Knowledge of Legislation and Guidance that impacts the NHS and Local Authority.
  • Aware of and Adheres to NMC guidance of Code of Conduct and Scope of Practice
