Continuing Healthcare Nurse Assessor

Job overview

The post holder will be responsible for the assessment of the individuals in the community/ home/ nursing homes and hospitals to determine eligibility to receive NHS Continuing Healthcare or Funding Nursing Care. The assessor will act as a member of the multidisciplinary team to co-ordinate the process, establish eligibility and assist in identifying a suitable care package. The assessor will be responsible to ensure the review process for all NHS Funded Care is undertaken in accordance with current guidance.  

Main duties of the job

The practitioner will utilise the national assessment tools and adhere to the required timescales, co-ordinate, participate and support the assessment process with the multidisciplinary team. The role will be part of the MDT and determine the individuals eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care. This will encompass explaining the process to patients and families; health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties.

Detailed job description and main responsibilities

  • To co-ordinate and undertake assessments. To collate information and assessments, gathered from other Health and Social Care professionals, patients and carers and make judgements using this information. Taking into account opinions from medical staff, consultants and specialist practitioners. This may involve complex and sensitive health and social care issues, patient’s diagnosis and prognosis. To maintain appropriate written records and communicate information to all relevant parties whilst maintaining appropriate confidentiality of the patient record.
  •  To restore and maintain independence, promote individual choice and self – determination, enabling clients to live in the community and supporting their preferences, whenever feasible. The use of Personal Health Budgets will assist in supporting this objective and will be delivered by the practitioner.
  • Maintaining a caseload of patients eligible for Funded Nursing Care and Fully Funded Continuing Healthcare, including the assessment and reviewing of all care provision.
  • To educate, inform and involve clients’ carers and professionals on the National Framework for Continuing Health Care and NHS Funded Nursing Care (2018) and matters of local service provision.
  • To be actively involved in complaint investigations assisting the Lead Nurse and Head of Service as required.
  • Contribute in the response to appeals supporting the Lead Nurse in completing needs portraits and attending both local and regional appeal panels.
  • Ensure the Head of Department, Lead Nurse and the Commissioning team are kept fully aware of any cause for concern in any care environment; liaising as required with the CCG Quality Team.

Qualifications

Essential criteria
  • RGN/ RMN- First Level Nurse
  • Evidence of on-going post-basic qualifications in area identified
  • Highly skilled clinical nurse competent to practice within expanded role
  • Educated to degree level (or equivalent/ evidence of working to)
Desirable criteria
  • Evidence of commitment to lifelong academic learning.

Experience

Essential criteria
  • Minimum of 2 years’ experience at Grade 6 level in related field.
  • Demonstrate comprehensive understanding of the Continuing Care and Free Nursing Care Agenda.
  • Proven ability to work across organisational boundaries.
Desirable criteria
  • Operational management in an Intermediate Care setting.

Knowledge

Essential criteria
  • Excellent verbal, written and presentation skills.
  • Computer literate- ability to use software programmes to maximise contribution to the post.
  • Demonstrate knowledge of current legislative changes affecting the NHS and the Local Authority.
  • Application of the NMC Code of Conduct and Scope of Professional Practice.
Desirable criteria
  • Project management skills to produce outputs within set timescales.
