Job overview
To complement the existing Multi-Disciplinary Team (MDT) in ensuring the provision of MDT meetings facilitation within the localities, assisting them with delivering effective coordinated care, thus maximizing the health and quality of life of the patient/client and improving the quality and efficiency of health/social care.

Main duties of the job
Working in conjunction with the Place Based locality model to co-ordinate ongoing patient/client case management.
Documenting and monitoring all aspects of patient/client care coordination and service delivery.
Collaborating and communicating with partners and wider health care teams to ensure the provision of appropriate services at the right time and in the right place.
To organise and administer the MDT meetings including presenting information, taking and distributing minutes to minimise the demands upon the GP practice and multidisciplinary team.
To attend Hospital and Specialist MDT meetings if required in support of patient identification and to improve care planning.

Detailed job description and main responsibilities
Act as a key role in the smooth running of integrated working within the multidisciplinary team setting.
Co-ordinate care by providing a single point of access for referrals into the MDT network.
Diary management of MDT meetings and other meetings in relation to patient/client co-ordination.
Manage agenda items, ensuring all new referrals and cases for discussion are identified and information circulated to team members in advance of the meetings.
To organise, take comprehensive minutes and administer MDT meetings including presenting information, ensuring timely dispatch of papers, and follow up of actions of meetings, informing GPs of actions from meetings to minimise the demands upon the GP practice.
To ensure that action points identified within the MDT are recorded and followed up.
To ensure that all patients/clients Care Plans, diagnostics results, and associated correspondence are available at the MDT meetings, liaising with all agencies as appropriate, accessing IT systems to ensure relevant information is available.

Qualifications
Essential criteria
- NVQ Level 3/4 in appropriate discipline eg Business Administration
- Qualification in IT packages, ECDL or equivalent
Experience

- Evidence of experience of a range of administrative systems and software programmes e.g. Word, Excel
- Previous experience in an NHS / Primary Care / Local Authority role
- Experience of planning and organising meetings preferably within multidisciplinary teams
- Experience of working within multi-professional teams
- Experience in use of data management
