Urgent Community Response - Clinical Support Worker

Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

Job overview
To support the Urgent Community Response service in the provision of care for housebound patients by assisting with clinical procedures and undertaking delegated duties unsupervised. This will involve direct care with clients (within their own homes, or care homes) who have been admitted to the care of the Community Urgent Care Response Service. The post holder will be supported and supervised by the senior clinician but will work regularly without supervision. This role supports the proactive monitoring of clients in order to promote and sustain their independence and to prevent unnecessary hospital admissions.

Main Responsibilities Clinical Responsibilities The post holder will highlight risk and changes in patient’s circumstances/condition by effective communication with registered practitioners.
•Be expected to implement and deliver a care plan, undertaking assigned procedures as directed, working within the boundaries identified by the registered practitioner.
•Demonstrate excellent communication skills with clients, relatives, carers and colleagues.
•Understand the roles of the multidisciplinary team and support services available within the community.
•Maintain excellent written and electronic patient records.
•To undertake nursing procedures and treatments as required, working within outlined lower-level clinical competencies and Trust protocols, as directed by the senior clinician.
•To work in partnership with patients, relatives, and carers and the wider multi- For One & All 2 disciplinary team in developing assessment, planning, implementation and evaluation of care.
•To support and enable clients to maintain their health needs.
•To deliver programmes of health education and health promotion as required.
•To undertake appropriate risk assessments and take the appropriate action which may avoid foreseeable harm occurring to patients in their own home.
