Walsall Healthcare NHS Trust provides integrated acute and community Healthcare in the West Midlands serving a population of 260,000 residents. Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in 2018, extension of a Neonatal Unit and an approved plan to build a brand new, state of the art Emergency Department which commenced in 2021

Job overview
An opportunity has arisen for an enthusiastic, motivated and experienced nurse with excellent communication to join the Diabetes Team. We are recruiting to a part time diabetes transition specialist nurse working closely with age groups between 16 and 25 years.
You will work closely with the Consultants, Clinical Nurse Specialist’s and other multidisciplinary team members playing an important role in the delivery of care and management of people with diabetes both in the community and acute setting.
Based between Bentley Medical Centre and Manor hospital you must be able to work autonomously supporting this population across the borough of Walsall , organize your workload effectively and adapt to, as well as drive change at a local level, being flexible is key. You should be a registered nurse or desirable registered children’s nurse with extensive post registration experience in nursing. A post graduate qualification in diabetes or experience in diabetes clinical care would be preferred.

- To work autonomously in the provision of specialist, evidence based diabetes care with a focus on the 16 to 25 year olds

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. A new Urgent and Emergency Care Centre, opened in March 2023, has significantly improved emergency care facilities and capacity, providing almost 5,000 square metres of additional clinical space.
Detailed job description and main responsibilities
To provide care in both the acute and community setting for people with diabetes including clinics in GP surgeries, health centres and patient homes
To work within a multidisciplinary team alongside consultants, nurses, podiatrists, dietitians and educators
To deliver both patient and health care professional education sessions in accordance with the service level agreement.

Essential criteria
- Registered General Nurse (with extensive post graduate clinical experience)
- Degree in nursing or related subject
- Post-basic qualification in children’s diabetes care in community or relevant subject or working towards
Experience
Essential criteria
- Relevant post registration experience, at Band 5 or above, working with children with diabetes
- Experience of multi-professional working
- Experience of working in an community setting, patient homes, schools
Skills
Essential criteria
- Good interpersonal and communication skills, both written and verbal
