ADULT VIRTUAL WARD /HOSPITAL AT HOME TEAM &INFUSION SUITE SERVICE

As a Band 6 Junior sister/ charge nurse at the Virtual Ward you will be expected to manage daily operations of caseload, triage referrals for admission, support staff with patient escalation for acutely unwell patients who would otherwise be in a hospital bed . Troubleshoot, safety net and make clinical decisions to keep patients safe at home.
We currently have a well established Adult virtual ward. You will be pivotal in supporting the implementation of expanding our current pathways and implementing new pathways in adult virtual wards.
Please note this vacancy role requires face to face patient management as well as virtual and therefore you will be expected to conduct face to face assessments . You will provide managerial leadership to the nursing team and also be part of working on our newly developed infusion suite .
Proficiency in IT and the ability to quickly learn multiple computer systems are crucial for this position.
Main duties of the job
To provide high quality nursing care and participate in the assessment, planning, implementation and evaluation of the care needs for the patient group, including leading and participating in MDT's to review and formulate treatment and discharge plans.
To deliver compassionate person-centred care, and treat patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family.
To provide relevant forms of care with minimal supervision and participate in the supervision and teaching and assessment of registered, unregistered, junior staff and students. In addition, you will be responsible for managing junior staff.
To play an active part in the continuing development of the ward or department and contribute to the maintenance of an environment conducive to the delivery of high standards of patient care, by promoting good communication and relationships between staff, patients, relatives and visitors.
Proactively identify suitable patients for the Virtual Ward including in-reaching within the acute hospital setting.
Providing IV medications to patients with a home environment and clinic/outpatient setting to avoid hospital admission.

Detailed job description and main responsibilities
To ensure safe and effective clinical practice.
Practice within an ethical framework based upon respect for the well-being and safety of patients and clients.
To be responsible and professionally accountable for the assessment of care needs; development of personalised plans of care, implementation and evaluation of care without supervision.
Lead a team and co-ordinate the clinical area in the absence of the Clinical Service Lead / Service Delivery Manager and reinforce the values and standards of compassion, dignity and respect in care, leading the delivery of clinically excellent care
Ensuring the planning and delivery of care is aligned to professional values of care, compassion and respect, act as a change agent, developing clinically effective practice through the effective utilisation and integration of evidence based practice.
Support the Clinical Service Lead / Service Delivery Manager in ensuring a culture of protecting patients from harm and deprivation of basic rights ensuring a safe and effective service with continuous quality improvement.
Promote a clean and safe environment for staff, patients and visitors by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical governance, including risk management and critical incident reporting and root cause analysis.
Ensure a high standard of care and record keeping in accordance with Nursing and Midwifery Council, Health and Care Professions Council, national legislation and local standards.
Demonstrating safe and effective care including; timely administration of medicines; effective observation of patients and demonstrate effective communication with the multi-professional team regarding patient care.
Assess the risks involved in the care of patients and ensure utilisation of practices and protocols to minimise those risks, including safe use of medical devices. To monitor the standards of care and with the support of the Senior Nurse and take appropriate actions when standards fall below expected levels.
Report incidents and near misses promptly and appropriately and with the support of the Clinical Service Lead/Service Delivery Manager and take effective action to minimise future risk. Contribute to the development and implementation of evidence-based protocols, standards, policies and clinical guidelines.
Critically appraise and synthesise the outcomes of relevant research, evaluations and audits and apply information when seeking to improve practice
To undertake e-triage of referrals in an efficient and effective way in line with triage protocols and identify suitable patients from the acute.
To maintain up to date clinical knowledge and experience in order to manage referrals in the most appropriate manner.

Qualifications
Essential criteria
- Active NMC registration
- Full UK driving licence and access to own vehicle
- Substantial experience as Band 5 registered nurse
Desirable criteria
- Current band 6 sister/ charge nurse in a setting caring for acutely unwell patients
- Health assessment qualification (level 7)
- V300 non-medical prescribing qualification
- Degree module for long term condition i.e respiratory, asthma, diabetes
Experience
Essential criteria
- Experience of long term condition management
- Experience in caring for patients who are acutely unwell
- Competent in cannulation and venepuncture
