Job overview
We are looking for an Advanced Clinical Practitioner to join our Virtual Ward and Hospital at Home Service. Our service is rapidly expanding and we are looking to further enhance the acute care that we deliver. This will be a fantastic opportunity to be part of a new venture, continuing to develop innovative care pathways utilising remote technology to provide acute care at home.
As an Advanced Clinical Practitioner you will take a lead in advanced assessment, prescribing, management and treatment of patients.
You will be based within the Virtual Ward and Hospital at Home service and work with part of the multidisciplinary team across primary, secondary and acute services. The aim of virtual ward is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients’, and their carers’ lives. This means we can proactively manage people such as those living with long term conditions and frailty at home, for those that have a sub-acute episode we can improve a patient’s experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.
You will be responsible for coordinating the provision of specialist interventions to meet the urgent health care needs of people who would have traditionally been hospitalised or those requiring facilitated timely discharge from emergency departments and step down from acute wards.
Main duties of the job
Delivering evidence based advanced practice skills to ensure the appropriate management of patients needs within the Virtual Ward and Hospital at Home Service.
Using skills of assessment, diagnosis, interpretation of tests, forward planning of care & treatment.
Review of newly admitted patients into the Virtual Ward and daily reviews including participating with speciality teams reviews and MDT's.
Timely recognition of deteriorating patients, appropriate response, escalation & documentation.
Routine writing of drug charts to prescribe medication for patients whilst in the Virtual Ward and in preparation for discharge.
Participate in, initiate, and deliver audit & research to identify current practice, drive continuous improvement & contribute to the development of new knowledge, theory & practice to benefit patients & colleagues.
Review of patients within the Virtual Ward and Hospital at Home and supporting the initial assessment of patients in the high acute beds (this will include in their own homes of patients with a range of conditions within various specialties.
Flexible approach to working within the Virtual Ward, Community Urgent Care and within the Accident and Emergency Department (front-door model) to step-up/step-down patients.
