Job overview

An exciting opportunity has arisen to join the Community Anticoagulation Service.

Do you have an interest  or previous experience working with patients who require anticoagulant medication?  or previous experience within Anticoagulation? 

 

Main duties of the job

Whilst community nursing/ anticoagulation experience is not required as we are committed to supporting your development during your induction and beyond. It is advantageous  if you have experience in  cardiology or renal nursing,  although  not essential. 

We are a  specialized service working towards enhancing care provision for people on an anticoagulant medication  and our current role is around education, monitoring, dosing and supporting patients  whilst on anticoagulant treatment. We also offer VTE/ in reach support at New cross, West Park and Cannock Hospitals.  As part of the service we offer  admission avoidance with administration of essential medications within the community to prevent hospital attendance/ admission for patients.  There are  exciting opportunities within the service to be involved with our self testing project and switching project which aims to maintain patient safety and offers a better experience for them, this is one of our quality initiatives. 

 There are also progression opportunities within the service to support professional and personal development, which  include but are not limited to completion of the  health assessment course and development into a band 6 role and opportunities to complete V300 course. 

Within this role, you will independently run a variety of clinics, completing new patient education and reviews, monitor patients on Warfarin and direct oral anticoagulants to maintain the correct doses/ treatment for their condition.
