Job overview

This is an exciting opportunity to join the Integrated Discharge Service (IDS) as IDS Matron/Service Manager on a full time basis (37.5hrs/week). This is a key leadership role within our dynamic and expanding service, offering the chance to make a significant impact on patient care and discharge processes across the Trust.

You will provide clinical and professional managerial leadership to the IDS, leading patient discharge daily and change management projects across the hospital. Working from our Transfer of Care Hub, you'll support our team of 5 IDS Operational Leads and wider service, ensuring quality, safety, and improved patient flow.

The IDS has experienced significant growth to meet increasing demands, and this senior role is central to our continued development and success. You'll be visible and accessible across the trust, acting as a role model while supporting patients, carers and staff. You'll ensure Trust policies are followed, implement robust governance systems, and communicate effectively with all levels of staff both internally and externally.

This role offers tremendous potential for an experienced leader to shape the future of discharge services within an innovative, collaborative team that is making a real difference to patient care and experience across the system.

Main duties of the job

  • Line management and supervision for a team of 5 IDS Operational Leads
  • Escalation point for clinical and HR situations from the Operational Leads
  • Training, development, support for the Operational Leads
  • Assurance on behalf of IDS that discharge delays are appropriately and effectively managed across the Trust
  • Input into strategic decisions that influence IDS
  • Day-to-day operational alignment of the IDS teams and staff groups
  • Day-to-day operational implementation of strategic plans for the IDS
  • Complaint oversight and assurance on behalf of IDS
  • Ensuring that the IDS are closely linked with the other partners in the Transfer of Care Hub and benefits are being realised
  • Deputising for the Head of Integrated Discharge and the Associate Director for Integrated Discharge where required
  • Input into Discharge to Assess programme work that involves the IDS, and attendance where necessary at system working groups and meetings
  • Leading patient escalation meetings on behalf of the IDS and NBT

Education & Qualifications

Essential criteria
  • Current NMC/HCPC/Social Work England Registration
  • Degree Level academic qualification or Equivalent in relevant field
Desirable criteria
  • Postgraduate clinical specialism and/or leadership/management qualification

Experience

Essential criteria
  • Minimum 5 years post-graduate experience
  • Over 2 years leading a clinical service/team
  • Evidence of collaborative working and network maintenance
  • Evidence of managing complex/difficult situations
Desirable criteria
  • Experience leading teams in an Integrated Discharge Service or similar structure
  • Experience leading change projects

Knowledge & Skills

Essential criteria
  • Have specialist knowledge of and ability to advise on the system approach to discharge management/processes
  • Good understanding of relevant legislation and statutory guidance
  • Ability to role model and lead collaboration across boundaries
  • Ability to assess, analyse and act on data to achieve change
