Trosolwg o'r swydd
The role is a key clinical leadership role focused on delivering physiotherapy and case management for patients with complex needs within community discharge hubs, interim health-funded beds, and inpatient setting on Chartridge Ward. The postholder will lead on holistic assessment, discharge planning, and collaborative care, ensuring timely and safe transitions of care. They will also support the wider multi-disciplinary team (MDT) and deputise for the Clinical Operational Lead when required.
Prif ddyletswyddau'r swydd
- · Provide advanced physiotherapy input across all Community Discharge Hubs, Interim Health Funded (IHF) beds, BHT hospitals, and other locality-based inpatient settings to support the delivery of a responsive and efficient discharge hub service.
- · Deliver skilled, holistic assessments and case management for patients with complex health and rehabilitation needs, ensuring accurate identification of ongoing care and therapy requirements.
- · Proactively case manage patients within complex health and rehabilitation needs in designated bed bases, creating and implementing person-centred discharge plans that optimise outcomes and reduce readmission risk.
- · Lead by example in intensive case management, holding responsibility for a defined caseload of patients with highly complex needs, while also supporting the wider team in managing similar patients.
- · Act as a resource and educator for members of the multi-disciplinary team (MDT), helping to develop their ability to deliver proactive and intensive case management and discharge planning
- · To develop and evaluate systems and processes that support enhanced case management across health and care services.
- · Engage and collaborate with external partners and agencies (e.g., social care, voluntary sector, primary care) to ensure smooth and safe discharge planning and to prevent failed or delayed discharges.

- Minimum of 3-4 years working experience at Band 6 level in the NHS
- Relevant post registration specialist/clinical experience and competence
- Experience in student clinical supervision
- Experience of working as advanced practitioner/case manager
- Experience in supervision of junior staff
- In-depth experience of interagency and partnership working
- Professional practice and leadership including day-today staff managemen
- Knowledge and application of evidence-based practice and clinical reasoning
- Leading complex care co-ordination

Education, Qualifications & Training

- Registered Allied Healthcare Professional with HCPC
- Educated to Diploma / Degree in Physiotherapy
- Relevant post graduate clinical training

- Ability to meet deadlines and work under pressure
