Job overview

To work as a member of the Urgent Community Response (UCR) and Frailty Hospital at Home (FH@H) team that provide rapid 0-2 hour responses to support patients at home who are in crisis and avoid inappropriate admission into hospital. 

Hospital level care can be provided in patient’s own homes, or usual place of residence, as part of a virtual ward, acting as a safe alternative to inpatient hospital stays helping patients to recover in their usual familiar surroundings.

The team is comprised of consultants, advanced clinical practitioners, nurses, paramedics, physiotherapists, occupational therapists and healthcare assistants. The role will provide nursing, physiotherapy and occupational therapy based interventions for patients.

It is a countywide role.

A full driving licence with business insurance and access to a car for work purposes is essential 

Main duties of the job

• Providing general and specific care to adults within competencies
• Being an integral member of the Urgent Community Response and Frailty Hospital at Home integrated team, providing defined delivery of care including core therapy, and nursing care to patients in liaison with clinicians’
• Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients’ own homes.
• Being responsible for the completion and potential delegation of non-clinical tasks within the team.
• Improve the care and support of patients in the community, so patients can remain safely in their own home for as long as possible
• Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
• Willingness to further develop your clinical expertise in a range of areas.
