An exciting opportunity has arisen for an aspiring individual to develop their career in the specialised area of District nurse. This opportunity is now available in the Chalfont's and Gerrards Cross area to become a Community Nurse within a newly formed team. We are welcoming highly motivated and effective team players, committed to providing a high-quality nursing service within the District Nursing field. We can offer a full skills development and preceptorship programme.

The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the speciality.
The post holder is responsible for all aspects of care including the assessment of care needs, planning and implementation / evaluation of care.
The post holder will be expected to carry out all relevant fundamentals forms of care with or without direct supervision maintaining a high standard of nursing care within the framework of Trust policies and in accordance with guidelines as laid down by the NMC.
To support the Charge Nurse/ Sister to monitor the safety and quality of care for patients.
In the absence of the Deputy Sister/Deputy Charge Nurse to be responsible for the day-to-day management of the ward with support from the Matron as required.
To manage a group of Nursing Assistants and assign delegated tasks and responsibility, as appropriate.
To ensure the compliance of mandatory training and appraisal requirements for delegated Nursing Assistants
