This opportunity is now available in the Chalfont's and Gerrards Cross area to become a Community Nurse within a newly formed team. The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the speciality.

The post holder is responsible for all aspects of care including the assessment of care needs, planning and implementation / evaluation of care.

 The post holder will be expected to carry out all relevant fundamentals forms of care with or without direct supervision maintaining a high standard of nursing care within the framework of Trust policies and in accordance with guidelines as laid down by the NMC.

To support the Charge Nurse/ Sister to monitor the safety and quality of care for patients.

 In the absence of the Deputy Sister/Deputy Charge Nurse to be responsible for the day-to-day management of the ward with support from the Matron as required.

 To manage a group of Nursing Assistants and assign delegated tasks and responsibility, as appropriate.

 To ensure the compliance of mandatory training and appraisal requirements for delegated Nursing Assistants
