Job overview
An exciting opportunity has arisen for a qualified Advanced Clinical Practitioner who has recent experience in Primary Care, to work in our Urgent Treatment Centre.
We require a forward thinking, well organised and motivated individual with a positive attitude.
Interested applicants must be a qualified ACP and hold core advanced practice competencies. Working as an Independent Non-Medical Prescriber is an essential.
The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients.
You will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. You will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, prescribing, health promotion, clinical decision making, planning of care and safe discharge of our patients within an agreed scope of practice.
You will be expected to work under the four pillars of advanced practice and be able to provide evidence of ongoing CPD in these pillars at appraisal.

Main duties of the job
The post holder candidate will be practicing in an independent manner within the Urgent Treatment Centre to provide patient-centred quality care for those patients with primary care presentations. This will encompass the skills of history taking, extended assessment, examination, diagnosis and treatment within an agreed extended scope of practice whist provided definitive care with limited requirement for supervision.

Qualifications
Essential criteria
- Relevant registered professional qualification (eg NMC/HCPC)
- MSc in Advanced clinical practice which includes o Advanced Assessment, diagnosis, referral and discharge o Assessing and managing risk o Critical thinking and analytical skills o Decision making/clinical judgement and problem solving o Higher levels of autonomy o Developing therapeutic interventions to improve service user outcomes o Advanced psychomotor skills o Higher level communication skills o Managing complexity

Knowledge / Experience
Essential criteria
- Significant urgent and primary care experience (Minimum 18 months)
- Experience of working as an autonomous practitioner in out of hours of Primary Care.
- Significant experience of working in primary care as a highly autonomous practitioner
- Established Advanced Clinical Practitioner
- Portfolio of evidence showing skills competence and achievements in practice/speciality
- Evidence of ability and knowledge on how to undertake audit and feedback
- Implementation of audit/research recommendations
- Evidence of audits undertaken and outcomes
- Clear understanding of urgent and emergency care performance targets and ability to identify opportunities to improve service delivery
- In-depth understanding of quality issues including clinical governance.
Desirable criteria
- Relevant experience of service transformation.
- Evidence of presentations at local / national levels
- Experience in recruitment and retention matters, staff appraisal and performance management
- Commitment to the on-going implementation of the Trust strategy.
- Evidence of involvement with primary research.
