CUH’s long term strategy relies on a significant change in the way in which we deliver care, and an increasing shift towards neighbourhood working. We are excited to be able to offer new roles for two experienced General Practitioners to work closely with hospital and primary care colleagues to design, test and scale new models of integrated care.

In these roles, you will provide senior primary care clinical leadership for CUH’s Neighbourhood Health Programme, shaping the development and delivery of new models of care across planned and proactive, and urgent and emergency care.

Main duties of the job

Two roles are available:

·        Primary Care Clinical Director for Neighbourhood Planned and Proactive Care,   AND

·       Primary Care Clinical Director for Neighbourhood Urgent and Emergency Care

The posts are for 2 years in the first instance. The post holders will each be allocated up to two sessions (8 hours) per week to undertake these roles, of which at least 6 hours should be during the normal working week in order to attend key meetings.

The successful candidates will join a small multidisciplinary team which also includes CUH clinical roles. This is an exciting opportunity to test, prove and scale genuinely innovative models of care which improve experiences and outcomes for patients, and make care easier to deliver for staff.

The roles will involve working with a wide range of colleagues across CUH and primary care, and through the South Care Partnership (representing health and care partners across Cambridgeshire). In particular, the post holders will be expected to work with South Place primary care leads, PCN Clinical Directors, the LMC and the wider GP body.
