We invite you to apply for the Band 5 Mental Health Practitioner position in the South Memory Assessment Service, Cambridge and Ely.  We are looking for someone with a keen interest in dementia and older people’s care. This post is open to registered nurses with the right attributes to enhance the developing Memory Assessment Service.

You will be contributing to the assessment of memory functioning and participating in the treatment process including post-diagnostic support. You will support service users and carers, offering information and signposting them to other agencies. You will work closely with colleagues from the multi-disciplinary team, psychiatrists, nurses, psychologists and others. We will provide the necessary support to ensure your development within your role

Main duties of the job

To work within the integrated multi-professional team and the Memory Assessment Service.

To be accountable for contributing to assessing, interpreting, planning and implementing treatment and care to patients within professional guidelines, with the support of the Band 6 mental health practitioner.

To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.

Provide support and education to peers, new staff, non-registered staff and Students.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Detailed job description and main responsibilities

 1.     To work within the integrated care team-mental health memory service to ensure that patients’ needs are met.

2.     To contribute to the assessment of memory functioning and health and social care needs, interpreting, planning and implementing treatment and care to patients within professional guidelines, with the support of Band 6 mental health practitioner.

3.     Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.

4.     To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.

5.     To support the band 6 mental health practitioner with interventions including support and guidance to both the person with dementia and their carer.

6.     Organise and manage own time allocating work appropriately. 

7.     To ensure that record keeping is to a consistently high standard

8.     To use evidence-based practice, to ensure clinical care is effective and appropriate.

9.     To highlight and report appropriate and potential risks.

10.  To ensure that they contribute and work towards the service/organisational aims and objectives of the Neighbourhood teams and the new model of care

11.  To ensure that accurate and timely written records are kept which comply with the Trust policy and NMC guidance, reporting on any issues as appropriate. 

12.  To ensure that they comply with current good practice in informing/updating all members of the multi-disciplinary team, their colleagues, service users and appropriate others of changes involving current nursing care plans, patients progress and other relevant matters that pertain to the care of the patient

13.  Provide support and education to peers, new staff non-registered staff and students

14.  To support the band 6 with day-to-day planning and delivery of a high quality nursing service.

Person specification

Education/Qualifications

Essential criteria
  • RMN/ RN/ NMC Registered Nurse
  • Evidence of professional development
Desirable criteria
  • ENB 998 or other teaching qualification, mentorship

Experience

Essential criteria
  • Experience in acute or community mental health setting – can be part of your training.
  • Experience of working with people with a diagnosis of dementia – can be part of training.
Desirable criteria
  • Community experience
  • Primary care experience

  • Able to demonstrate commitment to high quality care and service provision
  • Ability to work flexibly as part of a team
