The service supports working-age adults who have severe affective and psychotic disorders. The teams offer support in a number of ways, drawing from the expertise of such a variety of disciplines. 

To be right for this job: 

You  have a degree or relevant professional registration as a Nurse, Occupational Therapist or Social worker and will be an active member of a dynamic, highly motivated, skilled and creative team.

Prif ddyletswyddau'r swydd

The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure  and with a desire for innovation. We are able to support agile working for staff, using all appropriate I.T equipment and support, and have a secure area for staff bicycles. The city provides a good opportunity to be active at work, with staff being able to cycle to local appointments.

You will be involved in the delivery of care-coordination to a specified caseload operating under the CPA framework, a daily duty service, ongoing risk management, care planning, support around benefits and housing, carers’ assessments, physical health care and a wide variety of therapeutic interventions for which you can access training and supervision. You will also be involved in ensuring that the service is a positive learning environment for students from various disciplines.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To undertake assessment of individuals and their families/carers accessing the service with complex health presentations, including those service users presenting with higher levels of risk.

To develop alongside the service user, their care plan.

To offer specialist professional assessment and advice where required.

To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.

To regularly review risk factors and make changed to the management of them as necessary.

To work using assertive outreach approaches to engage the service user and their families.

To contribute to the maintenance and development of the Trust nursing strategy. 

To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.

To provide supervision of junior staff and trainees where appropriate.

To act within the professional code of conduct at all times.

  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role OR, Registered Social Worker OR, Registered OT.
  • Evidence of continuous professional development

Experience

  • Significant post qualifying experience in working with those presenting with serious mental illness
  • Experience of working alongside people with complex health problems in the designated care group
  • Experience of supervising staff
  • Experience of managing a caseload
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of conducting clinical assessments including risk assessments
  • Experience and knowledge of dual diagnosis/substance misuse
  • Previous experience of working in the community
