Stroke ESD and Community Neuro Physiotherapist

Band 5

Job overview

We are seeking an enthusiastic Band 5 Physiotherapist with previous NHS experience & prior involvement in stroke rehabilitation to join the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service supporting service users with a variety of complex needs.

ESD is a key pathway for stroke survivors allowing them to leave hospital earlier than they may have otherwise, receiving up to 6 weeks of intensive rehab in their home, community or our inpatient Community wards.

The successful candidate will manage a clinical caseload across the South of Cambridgeshire. their base being either Huntingdon or Cambridge. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire alongside our established multi-disciplinary team of Occupational Therapists, Physiotherapists, Speech & Language Therapists, Psychologists, Specialist Nurses and Neuro Rehab assistants. The role requires agile working often using virtual consultation to manage a caseload.

Main duties of the job

You will provide comprehensive assessments and deliver evidence-based specialist neuro physiotherapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential. Given the specialist nature of this service, applicants must bring prior experience of stroke rehabilitation—whether gained in acute, community, ESD, or neurorehabilitation settings—to ensure confidence and competence when working with this patient group.

We pride ourselves on the development of our workforce and provide a variety of internal and external training opportunities. The successful post holder will receive and provide clinical supervision and an annual appraisal.

The ideal candidate should be flexible and adaptable to support service delivery. The successful post holder must be able to travel independently across their work locality, have a valid driving licence and access to a car for work purposes.

To achieve our goal, we look to recruit high-calibre candidates who share our vision and values. As an equal opportunities employer, we encourage applications from all sectors of the community, particularly from under-represented groups including people with long term conditions and members of our ethnic minority and LGBTQ+ communities.
