Job overview

As a Secure Case Manager for the EoE Provider Collaborative, you will be working as part of the Quality Team to support the pathways for service users through secure hospital care. 

You will be responsible for a caseload of adults who are inpatients in low, medium and high secure services and will lead on the quality oversight of a number of secure hospitals sites within the region. 

Main duties of the job

The case manager will have responsibility for oversight of the pathway of adults through low, medium and high secure services. Although the majority of placements are within the East of England, some service users are placed outside of the region in specialist services across the country and the case manager is expected to visit all individuals on an 8 weekly basis.

The post holder will also be responsible for leading on quality oversight of a number of secure hospitals sites within the region and will carry out regular visits as well as annual formal service reviews.

As a clinician within the TACT the post holder will work closely with the commissioning and transformation teams to ensure continuous improvement of the service.

Detailed job description and main responsibilities

• To provide clinical assurance of the quality of the care provided by specialised services within the east of England.
• Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved.
• Provide proactive case management through effective liaison with significant stakeholders for the placement of patients according to evidenced clinical need and undertake monitoring of the placement through regular attendance at relevant CPA meetings and Section 117 reviews to ensure the effectiveness of treatment and efficacy of care packages.
• To be involved in patient forums in order to appreciate the patient experience and address, with the Senior Case Manager and Commissioners, issues raised with regards to compliments, concerns and complaints.
• Attend provider’s referral and bed state meetings.
• Work collaboratively with all providers to ensure quality improvement processes are in place.
• In the event of a serious and untoward incident being reported by a provider, the post holder may be required to investigate and report to the Senior Case Manager on the incident and actions.
• Take an active role in the development of partnership working across the east of England to promote timely and equitable patient access to services once assessed as having a clinical need and risk, and timely egress from services ensuring the most appropriate level of security/setting is available in accordance with the patients agreed care pathway. Identify and negotiate placements for patients at the point of initial referral.
• Establish and maintain effective links and good communication with all relevant specialised mental health and learning disability service providers (NHS and Independent Sector), Clinical Commissioning Groups (CCGs), prisons and other key stakeholders who provide services to patients within the east of England.
• The post will also include being responsible for ensuring the development and articulation of best practice across providers of the pathway.
• Attendance and participation at local, regional and national forums related to specialised services.
• To work collaboratively across the NHS England and NHS Improvement matrix, including integrating the National Director’s portfolio.

Person specification

Education

Essential criteria
  • Registered Mental Health/Learning Disability Nurse or other registered health or social care professional
  • Further training in project management/financial management or supporting the change process
  • Educated to Post-graduate degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area

Experience

Essential criteria
  • Significant operational knowledge of secure services, including risk assessment/management
  • Knowledge of legal issues pertaining to the specialist service, including MHA 1983, Ministry of Justice procedures
  • Demonstrable specialist knowledge in a relevant discipline
Desirable criteria
  • Demonstrated experience in a Healthcare environment.
