An excellent opportunity for a very experienced, dynamic  clinician (Nurse/Occupational Therapist/Physiotherapist) to be part of a systemwide Long Covid Assessment Service. 

 

 

Main duties of the job

The successful candidate should have experience of working with patients with complex long term conditions and be competent at triaging and prioritising clinical need.

Detailed job description and main responsibilities

  • To work as a member of the Long Covid team to undertake advanced assessment, monitoring and evaluation of complex long Covid caseload; working seamlessly across primary and secondary care boundaries
  • Screening of the Long Covid caseload and management of the assessment of patients in a virtual Multi disciplinary clinic and complex case face to face clinic
  • The development of links with patients and other members of the multidisciplinary team to ensure patient pathways are robust and fit for purpose
  • Implementation of National and local policy directives in conjunction with clinical lead
  • Participation in multidisciplinary team (MDT) working
  • Provision of specialist advise and education to colleagues and other members of the MDT
  • To understand the strong sense and expectation of role and skill sharing with the specialist services.
  • To work collaboratively with colleagues across the system

Education / Qualifications

Essential criteria
  • Professional qualification at degree level and current registration
  • Post registration qualification
  • Evidence of continued professional development and its application
  • Evidence of research appreciation
Desirable criteria
  • Masters degree (or working towards this)
  • Leadership qualification
  • Teaching qualification or equivalent experience

Experience

Essential criteria
  • Evidence of strong multidisplinary & multi-agency working
  • Ability to work autonomously and use initiative
  • Understanding or wider NHS and social care issues
Desirable criteria
  • Community experience
  • Advanced communication course

Skills & Abilities

  • Able to demonstate commitment to high quality care & service provision
  • Broad range of clinical skills required for the post
