 We have an exciting opportunity to join our fast paced, dynamic Intermediate Care Team based in Peterborough.

The Intermediate Care Service offers care and therapy support to patients in the Cambridgeshire and Peterborough health system.  The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital and admission avoidance, using the Home First model.

Working in collaboration with the Clinical Team Lead, the post holder will offer leadership and management to Coordinators and Integrated Care workers within the locality team, ensuring the effective delivery of Intermediate Care as part of the countywide service. The post holder will ensure the smooth implementation of tasks and responsibilities assigned to care coordination regarding allocations, care planning, referrals, pending list management and patient flow.

The successful applicant will need to be innovative, decisive and display strong leadership and organisational skills.

Main duties of the job

To manage and co-ordinate a team of Care Coordinators and Integrated Care Workers, in order to ensure effective and efficient service delivery, development and quality assurance of the services provided within the team.

To work closely with Team Lead and therapy staff to promote an multi-disciplinary approach to management of patient care

To work collaboratively with colleagues across the social care, health and the third sector to provide patient centred, integrated care

To assist in the development and implementation of change to modernise and improve the service provision in the Intermediate Care Team

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.

To ensure care planning and allocation of intermediate care service for all services users within the intermediate care service comply with both the CGC regulations and clinical governance arrangements for the service.

To support ICWs to ensure that care plans are, maintained, updated and recorded in accordance with the service’s standard operating procedures and policies.

To line manage Assistant Care Co-ordinators and Integrated Care Workers within the Intermediate Care Service.

To work in conjunction Team Lead to, ensure data collection supports management information.

To ensure that vulnerable adults are safeguarded and promoting the welfare of children, young people and expects all staff and volunteers to share this commitment.

To ensure that sufficient staff are deployed with the required skills and experience to meet the service needs.

Person specification

Education

Essential criteria
  • Degree or NVQ IV/Diploma in Management plus relevant experience working in an intermediate care setting
  • Evidence of postgraduate Continuing Professional Development and its application
Desirable criteria
  • CQC registered manager

Experience

Essential criteria
  • Experience of recruiting, training, assessing and appraisal of staff
  • Experience of working in Community Care services
  • Experience of working with independent sector providers
  • Considerable experience of working with adults/older people with disabilities
Desirable criteria
  • Previous experience of managing staff in a Care service

Knowledge and Understanding

Essential criteria
  • Knowledge Safeguarding principles and promoting the welfare of vulnerable adults.
  • Knowledge/understanding of principles of good care practice

Skills and Abilities

Essential criteria
  • Excellent written and verbal communication skills
  • Organisational skills and ability to plan and prioritise, while working under pressure
  • Ability to use a variety of patient electronic record systems eg Cygnum, SystmOne
  • Problem solving skills, ability to confront issues and seek solutions
  • Good keyboard skills and ability to communicate through IT systems and use Word, Outlook and Excel
  • Ability to be proactive, provide strong leadership to staff and motive them in times of change
