Job overview

An Exciting opportunity has arisen in the Inflammatory Bowel Disease (IBD) Team in Cardiff and Vale UHB through service redesign to welcome Band 7 Clinical Nurse Specialists into the team to support the patients suffering with IBD. 

The service actively cares for over 4000 patients per year, supporting patients to self manage their condition, to stay at home and maintain a healthy lifestyle.  As well as operating a busy outpatient service for those patients with more complex presentations, including Hot Clinics, Advice lines, Self Management programmes, biologic therapy in our dedicated infusion suite.  The IBD service also offers advice to GP practices as well as to any inpatient areas in Cardiff and Vale UHB.

The post will require to work between the University Hospital of Wales and the University Hospital of Llandough with your main base being in Llandough Hospital.

The IBD Service is looking for an experienced IBD CNS or those with transferable skills to join the service to help lead and develop the service to provide the highest standard of care to our patients.  Whilst holding a Non-Medical Prescribing qualification is preferable, the service will support any successful candidate to achieve this qualification if needed.

We are looking for a dynamic, kind and responsive individual who is wanting to drive the IBD Service forward with the Lead Consultant and other Nursing team members. 

Main duties of the job

  • To competently provide highly specialist nursing knowledge, support and advice for patients with IBD, based on the assessment, development, implementation and evaluation of programmes of care.
  • Complex decision making and counselling of patients and their carers.
  • To co-ordinate recruit and manage patient on self-directed care.  Follow up and surveillance of patients at high risk of colon cancer and also those patients at risk of developing osteoporosis following steroid therapy.
  • To provide training and supervision to patients receiving Methotrexate/ biologic injections subcutaneously and to provide appropriate follow up for these patients
  • To organise and manage  Nurse-Led out-patient clinics, to assess newly diagnosed Inflammatory bowel disease patients in clinic independently, including undertaking Clinical Assessments and developing complex management plans, with the support of the Medical workforce. 
  • Perform extended nursing skills as required including cannulation, venepuncture, radiological requesting in accordance with UHB policy (Support with further education will be provided)
  • Management of the patient from diagnosis, on-going investigations and treatment changes will be made by the post-holder, including complex decision making, referring to more senior colleagues or Consultant when needed.
  • To support and engage with audit and research in the service.
