Job overview
Advanced Pharmacist to join the tertiary Intestinal Failure service at the University Hospital of Wales.
We have an exciting opportunity for a motivated, enthusiastic and experienced clinical pharmacist to join our patient-focused, progressive and supportive intestinal failure service.
The post holder will have the opportunity to provide prescribing, clinical pharmacy services and medicines management to patients under the highly specialised intestinal failure service for South Wales hosted by Cardiff and Vale University Health Board. They will play an integral role in directing these services as part of the MDT, ensuring provision of a sustainable, high quality pharmacy service. The post holder will drive change and improvement in both clinical outcomes and patient experience through service improvement projects, audit and research, and will have responsibility for liaison and collaboration with relevant services and key stakeholders. They will be accountable for provision of education and training to healthcare professionals on all aspects of medicines management in intestinal failure.
The post holder will be supported in developing their advanced practice by our experienced team of pharmacists and the excellent team of healthcare professionals working within the multidisciplinary team.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
This varied role involves participating in multidisciplinary ward rounds, outpatient clinics, and MDT meetings. In addition to clinical duties, the role includes a technical services element, where you will use your unique understanding of PN formulations to assist in the technical release of parenteral nutrition and chemotherapy. This position offers a great opportunity to develop your clinical leadership skills and gain broader experience in all aspects of managing complex nutritional support and intestinal failure.
