Job overview
Cardiff Community Resource Team is a community based multidisciplinary team consisting of Therapists, Nurses, Consultants and associated support staff. The team works to support people at home and either avoid hospital admission or facilitate early discharge, with rehabilitation being delivered within the person's own home. We work collaboratively with Cardiff Local Authority colleagues who provide domiciliary care when required. We work across the whole of the beautiful City of Cardiff to ensure support is delivered to people within their own communities and closer to home. As a rehabilitation coach you will be working specifically with patients who have had a fall, or who are at risk of falls.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The successful candidate will have qualifications relevant to the role and strong knowledge of falls prevention and falls management interventions. They will have previous experience in health promotion and demonstrate excellent communication and interpersonal skills.
Working under supervision of registered practitioners, they will organise and deliver community treatment and evidenced based falls programmes for identified patients in the locality. This will include physiotherapy, occupational therapy and dietetic interventions.
The candidate will be required to work in patients’ home in an unsupervised capacity whilst preparing and carrying out routine elements of patient triage and assessment. Additionally, the successful candidate will have to plan and take responsibility for care/treatment of the community patients they are accountable for.
Other requirements include monitoring patients and alerting therapists when there are unexpected changes, and modify approaches and activities within care/treatment using own clinical reasoning notify a registered practitioner accordingly, utilising evidenced based outcome measures where required.

Detailed job description and main responsibilities
Triage, assessment and delivery of treatment interventions for a specified group of patients in the community who have experienced a fall, or who are fearful of falls using structured tools, under supervision of registered staff
Delivery of individualised strength and balance exercise programmes
Prescription and provision of equipment
Management of own clinical caseload
Working in an office environment, and out in the community in patient homes
Participation in MDT discussions
Maintain accurate patient records in accordance with policy on PARIS IT system
Working collaboratively with other members of CRT
Having an excellent knowledge of, and working collaboratively with 3rd sector services
Collection of outcomes measures and other relevant data

Qualifications
Essential criteria
- Numerate and literate to GCSE level or equivalent
- Good command of English
- Recognised qualification NVQ level 3, Agored Cymru or equivalent.

- Evidence of further education e.g. relevant Health and Social Care qualification, sport science, or other relevant qualification
- Basic Food Hygiene
- Qualified or working towards a Level 4 Exercise Rehabilitation qualification
