Job overview

 

The Live Well service seeks a compassionate and motivated individual to join its therapy team as a Rehabilitation Coach.  

The postholder will be responsible for: 

  • Supporting people with long term Neurological conditions to meet their goals in a number of community settings including groups and 1:1 under the guidance of the registered staff.  

  • Leading group activities.  

  • Delivery of online and face to face interventions.  

  • Collection of data and service user administration.  

  • Applicants will have relevant qualifications to the role and knowledge of health promotion, long term condition management and experience of group facilitation.  Previous experience as a therapies support worker or experience as an exercise professional is advantageous. They will have experience in health promotion type activities and demonstrate strong communication and interpersonal skills. They will need to have excellent verbal and non-verbal communication skills, be able to work as part of a varied team, and be able to motivate service usersPrevious experience of groups delivery , data collection and audit are desirable.  

  • The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. 

 
 

 

Main duties of the job

JOB PURPOSE 

  • To provide a service that will promote physical and functional activity and promote healthy lifestyle for residents of Cardiff and the Vale UHB living with long term conditions.  

  • To work under supervision and assist the Live Well service Therapists in clinical and administrative duties.  

  • To be delegated routine interventions to be carried out independently, and to progress these interventions within guidelines laid down by the Live Well service Therapists.  

  • In addition, the post holder may be required to work in other areas depending on service needs.

Essential criteria
  • Numerate and literate to GCSE level or equivalent. Dealing with people individually or in groups
  • Recognised qualification NVQ level 3,Agored Cymru or equivalent.
