Job overview
The Rehabilitation Health & Care Support Worker delivers delegated care/tasks across a range of disciplines as part of a team based approach to delivering holistic patient centred care. Ystwyth ward specialises in Stroke and frailty rehabilitation.
A key aim of the role will be to support the patient to achieve optimum independence by assisting in the provision and monitoring of rehabilitation services; implementing an agreed individualised plan of care as determined by the team.
The post holder is expected to always treat individuals with dignity and respect recognising their wishes and beliefs empowering the patient to have choice and control of their care.
The post-holder will practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.
The post-holder will be expected to develop within the role as required within the boundaries of the job description and agreed competencies.
The post holder must communicate effectively at all times and in conjunction with other team members contributing to and maintaining the team/ward philosophy.
Main duties of the job
As a conscientious and enthusiastic individual, would you like to work within a specialist, supportive, dynamic, multidisciplinary team who are passionate about all aspects of rehabilitating post injury.
You will be supporting patients to transition from emergency or acute care to home. Working closely with therapists and nurses you will enable people to reach their individual goals and optimum functional baseline.
The ability to speak Welsh is desirable for this post: English and/or Welsh speakers are equally welcomed to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

- Relevant NVQ / Diploma at level 3 or equivalent experience and prepared to work towards achieving this level of qualification within two years of appointment
