Job overview

This is an exciting rotation opportunity for an experienced Occupational Therapist. We are looking for an Occupational Therapist to contribute to our Acute and Community settings in Pembrokeshire, contributing to improvement, development and change of culture, rotating at 9 months:-

Frailty Pathway - MDT working in the acute setting to avoid  unnecessary acute hospital admission, maximize independence and support timely discharge from hospital

Urgent and Immediate Care - supporting an intermediate care therapies model covering Rapid Response in the community, rehabilitation and admission avoidance.

Acute Medical and Trauma Rehabilitation  in General Hospital,  providing continuity of care for patients, promoting faster recovery from illness and assisting people to achieve their potential to manage at home.

Based in beautiful Pembrokeshire the successful candidate will be part of a friendly, proactive occupational therapy service, and will work collaboratively with multi-professional teams delivering high quality, evidence based, person centred occupational therapy.

Full time or part time available.

The Occupational Therapy Service has clinical networks and integrated pathways that span primary and secondary care and proactively supports training and development both as part of internal and external CPD.

Main duties of the job

Work as part of a  team to deliver an effective and efficient occupational therapy service to improve the health & wellbeing of people.

Deliver the service in a manner and setting appropriate to the needs of client group, following professional codes of conduct and ethics and national and/or locally agreed guidelines, protocols, policies and care pathways.

Working with colleagues or alone; with individuals and groups; in people’s homes, community settings and hospitals.

Autonomously manage an occupational therapy caseload, which includes those with complex presentations.

Selecting and undertaking assessments, developing person centred plans, delivering interventions, evaluating outcomes and adapting plan of care based on outcomes.

Work effectively as part of a multidisciplinary team to provide 
rehabilitation/recovery/reablement focussed on the needs of the individual.

Work collaboratively with colleagues from Health, Local Authorities and Third Sector Organisations to achieve best outcomes.

Support team operations, dealing appropriately with enquiries and undertaking administrative tasks relevant to the role.

Support the induction, education and development of others.

Support quality improvement, taking responsibility for defined projects.

Appropriately delegate work to, and supervise the work of, Junior staff, Students and Volunteers as required.

Working for our organisation

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

Detailed job description and main responsibilities

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The post holder will have current HCPC registration and will be expected to travel between sites in a timely manner to visit patients at home.

As 7-day services are being developed, applicants must be prepared to work as part of a rota over 7 days.

Mileage allowance is payable, pool cars are available.

Qualifications and Knowledge

Essential criteria
  • Degree in Occupational Therapy
  • Current HCPC Registration
  • Knowledge of relevant current health and social care legislation applicable to role
  • Knowledge and application of Microsoft Office software
  • Understanding of the roles of other staff groups in health and social care
  • Knowledge of a range of rehabilitation interventions across all therapies
  • Understanding and application of clinical governance, including research and audit methodology
  • Understanding and application of relevant outcome measures to demonstrate effectiveness
Desirable criteria
  • Knowledge of other services that link across primary/secondary/ intermediate care
  • Working Knowledge of therapeutic equipment
  • Knowledge of Health Board and priorities
  • Accredited Practice Educator
  • Accredited Agored Assessor

Experience

Essential criteria
  • Significant post graduate experience of working as an occupational therapist across a range of settings and variety of conditions
  • Managing a complex and varied caseload
  • Working independently with indirect or arms length supervision
  • Delegation and Supervision of staff
  • Experience of coping independently with distressing or emotional situations in challenging work environments
  • Working as part of a team
  • Experience of contributing to service development and implementing service changes
  • Direct involvement with carers and families
  • Self-development and application of learning
  • Use of adapted techniques for improving patient outcomes
Desirable criteria
  • Experience of working with groups
  • Experience of working in a community setting
  • Experience of using virtual communication platforms for delivering service
  • Experience in same clinical area as post being applied for
