Are you a dynamic, compassionate, and highly motivated nurse with advanced knowledge and skills in palliative care? If so, an exciting opportunity has arisen for a part-time  Band 6 Clinical Nurse Specialist to join the busy and dedicated Community Palliative Care Team (SPCT) in Pembrokeshire. 
 
This is a rewarding opportunity to be part of a team committed to improving the quality of life for individuals—and those closest to them—who are living with progressive, life-limiting illnesses. The role focuses on delivering expert symptom management, psychological support, and holistic care, with a strong emphasis on palliative and end-of-life care within the acute setting. 

Main duties of the job

Key Responsibilities:
  • Deliver high-quality, compassionate care and specialist advice to patients, families, and caregivers.
  • Communicate sensitively and effectively with patients, families, and multidisciplinary teams.
  • Support and educate colleagues to ensure holistic care planning, including advance care planning and DNACPR discussions.
  • Identify patients with palliative care needs early to prevent unnecessary delays in treatment 
  • Manage a defined caseload under the guidance of the Consultant, working closely with Clinical Nurse Specialists, doctors, therapists, and the wider community MDT.
  • Contribute to service improvement, education, audit, and research initiatives.
What We’re Looking For:
  • A registered nurse with significant experience in palliative care.
  • Excellent communication skills and emotional intelligence.
  • A passion for delivering proactive, person-centred care.
  • Ability to work both independently and as part of a multidisciplinary team across the community.
The ability to speak Welsh is desirable for this post; however, both English and Welsh speakers are encouraged to apply.

Qualifications and Knowledge

Essential criteria
  • Registered Nurse (Part 1)
  • Current NMC Registration
  • Bachelor’s Degree
  • Post registration/ postgraduate qualification in specialist area / evidence of post registration study at equivalent level Proficient level of knowledge in clinical speciality
  • Knowledge of service improvement
  • Able to undertake a literature search and critically appraise research papers
Desirable criteria
  • Teaching qualification (or equivalent experience)
  • Advanced clinical skills
  • Non-medical Prescribing (V300)

Experience

  • Experience in the relevant specialty
