Job overview

We have an exciting opportunity for a Band 5 Community Nurse to join our Community Nursing Team based in Milford Haven, Pembrokeshire.  

The role will allow you to support the management of a delegated caseload within the Community Nursing Team providing high quality and complex care management to patients in their own homes, with the guidance and support of an experienced District Nurse Team Leader.  

The post holder will ensure high standards of care are delivered to patients and their families/carers within the home environment. This includes liaising with staff, patient/carers and other agencies/organisations as required to ensure appropriate delegation of duties and risk management, whilst ensuring effective communication between those involved in the care package.

Main duties of the job

Pembrokeshire’s Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy. 

These are permanent positions, worked over a 7-day period including weekends and bank holidays. Full and part time opportunities are available.

The post holder will need the ability to travel between sites in a timely manner.

The ability to speak Welsh is desirable for this post: English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties. 

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services. 

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers. 

Our services are provided in: 

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest 

  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire 

  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings 

  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers 

  • Numerous mental health and learning disabilities services

Qualifications and Knowledge

Essential criteria
  • Level 1 Registered Nurse (Adult)
  • Current NMC Registration
  • Evidence of personal continuing development, statutory and mandatory training
  • Awareness of Health Board policies, procedures and guidelines
  • Ability to relate theory to practice
  • Understanding of primary health care working
Desirable criteria
  • Postregistration experience, in acute services
  • Teaching and assessing
  • Understanding of clinical governance, health & safety, risk management awareness
  • Community experience
  • Willing to undertake further training as necessary
  • Evidence of post-registration education – Diploma/ Degree Level

Experience
