Job overview

We are looking for a Clinical Lead to join our team with a range of sessional commitments and can offer a mix of face-to-face consultations, telephone triage and remote consultations.  The Practice uses E-Consult and has signed up to Workflow Optimization. You will be expected to demonstrate leadership skills across the clinical and management team, supporting practice development and innovation in service delivery.

You will be part of the wider Health Board GP team and have the opportunity to participate in a monthly Journal Club and have peer support through the Deputy Medical Director’s team, as well as from your salaried GP peers and from the Primary Care Cluster Locality Lead.  There is the opportunity for training and development in any areas of specialist interest.

Main duties of the job

Main Duties and Responsibilities

  • To provide, as part of a core multi-disciplinary team, a high quality general medical service to the registered patients of the practice (including temporary residents)
  • To provide consultation and appropriate clinical management of patients attending the practice, or to those where a home visit is clinically necessary
  • To provide leadership, clinical oversight and direction to the rest of the clinical team
  • To establish and maintain appropriate chronic disease management clinics
  • To provide telephone and/or virtual consultation and appropriate management of patients who contact the practice, where necessary
  • To undertake telephone triage as and when necessary
  • To keep accurate and legible records of all patient contacts and contribute to electronic data recording and audit as directed by the service.
  • To ensure in conjunction with the lead nurse and Practice Manager that all medical and other equipment is maintained in good and substantial repair and condition for effective use.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

· Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire; 

· 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres; 

· Numerous locations providing mental health and learning disabilities services; 

· Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales. 

Detailed job description and main responsibilities

Service Management Duties and Responsibilities

  • To lead in operational and strategic planning for the development of services and the practice
  • To develop and encourage positive working relationships with local Primary Care contractors, housing, social work, voluntary services, drug and alcohol, mental health, dental, secondary care and other appropriate services
  • To participate in development activity and projects as requested or required by the Health Board, including development/review of protocols, documentation, clinical governance issues, audit activity, training and research
  • To keep up to date with current and forthcoming clinical guidelines their impact on Primary Care Services and ensure their implementation within the practice
  • To support the development new services (including enhanced services) within a practice setting and thereby maximising service provision within the locality
  • To lead and maximise the quality agenda within the practice by using the Quality Assurance and Improvement Framework and other qualitative tools
  • To attend and contribute to professional collaborative, administration and clinical meetings.
  • To participate and contribute to the education and training of other professionals and colleagues within the practice team, undertaking clinical oversight and mentorship as required
  • To contribute to the smooth running of the practice by attending team meetings.
  • You will be responsible for undertaking the administrative duties associated with the care of patients and the administration of the practice.
  • You are required to comply with the appropriate Health Board Health and Safety Policies. As part of this, all employees have an individual responsibility to adhere to the Infection Control Policy and to protect themselves, the patient, visitors and colleagues from the risk of infection. The individual responsibility will include the requirement to attend training at intervals determined by the Health Board and understand Infection Control issues as they pertain to their workplace. They will report any identified infection risk and take necessary precautions/actions to prevent transmission.
  • Under the provisions of the Data Protection Act 1998 it is the responsibility of each member of staff to ensure that all personal data (information that is capable of identifying a living individual) relating to patients, staff and others to which they have access to in the course of employment is regarded as strictly confidential.  Staff must refer to the Health Board’s Data Protection Policy regarding their responsibilities.
  • Travel as necessary between sites will be required
  • You will be required to participate in and contribute to Clinical Audit in line with the Health Board’s policy on the implementation of Clinical Governance.
  • You will become a member of the South Pembrokeshire Cluster team and will be required to attend relevant meetings as appropriate.

Meddygfa Penrhyn/Peninsula Practice initially joined the Hywel Dda University Health Board portfolio of Managed Practices in April 2023, following resignation of the GMS contracts of two smaller practices. This is an opportunity to join a circa 4,700 patient Practice which currently operates from its main site in Solva as well as a nurse led branch in St Davids. You will be part of a multi-disciplinary team that includes GP’s, Advanced Nurse/Paramedic Practitioners, Pharmacist and Pharmacy Technician support.  The Health Board is committed to multi-disciplinary team working and will be looking to expand the team further in the coming months.

Meddygfa Penrhyn is situated in the North Pembrokeshire Cluster. The Practice has the opportunity to participate in all Cluster developments and initiatives and will be represented at the Professional Collaborative group as part of the Accelerated Cluster Development programme.

Hywel Dda University Health Board (UHB) includes the unitary authorities of Carmarthenshire, Ceredigion and Pembrokeshire and covers a quarter of the landmass of Wales and is the second most sparsely populated Health Board.  The North Pembrokeshire Cluster is made up of 8 GP practices. The economic base of the county is focused on agriculture (86% of land use), oil and gas, and tourism. Pembrokeshire beaches are award winning!  The Cluster is a busy tourist destination where the population of some towns can double in size during the holiday periods, with also a steady influx of visitors throughout the year.  The County hosts many popular sporting events all year around e.g., Iron Man and The Pembrokeshire Long Course Weekend. 

The Cluster serves a population that is older than the Welsh average; this has potentially the greatest impact on primary care with the increase of co-morbidity and chronic illness that will need active management.

Qualifications and Knowledge

Essential criteria
  • Entry on the GP Register
  • Full registration and a License to Practice with the General Medical Council
  • Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent due within 3 months of anticipated interview date
