An exciting opportunity has arisen for a Band 6 Community Nurse  Sister/Charge nurse to join the Tenby District Nursing Team.
 
This post does not require the Specialist Practitioner Qualification, in District Nursing or the need to work towards the qualification, as the post is a fixed term secondment for development.
 
This is a challenging yet rewarding  role requiring the successful applicant to combine excellent clinical practice with day-to-day shift co-ordination, in addition to undertaking leadership and management responsibilities in support of the District Nurse Team Leader.  You will be required to formally deputise in the absence of the District Nurse Team Leader.
 
This is a fixed term  position working  5 days over a 7-day period including weekends and bank holidays.
 
The post holder will need the ability to travel between sites in a timely manner.
 
This is a full-time post however part time will be considered. 
 

Main duties of the job

Post holder will be required to assess care needs and develop, implement and evaluate programmes of care without supervision. Ensuring high standards of care are delivered to patients and their families/carers within the home/clinic environment.
 
Delegate the delivery of care where appropriate, in line with NHS Wales delegation framework. Supervise the delivery of care and staff involved with its provision.
 
Ensure that the patient's physical, psychological, social and spiritual and fundamental aspects of care needs are met.
 
Be accessible in the community setting to the nursing and wider multi-disciplinary team, patients and service users within primary and secondary care.
 
Continually support and work in conjunction with the Senior Sister/ Charge Nurse and formally deputise in their absence.
 
Deal with complex clinical and managerial situations that arise, critically analysing, seeking appropriate help and making decisions in relation to patient risk and care.
 
Seek appropriate advice/alert agencies when there are any concerns regarding potential/actual risk to any patient or their dependents e.g. child safeguarding, domestic violence, protection of the vulnerable adult, mental capacity and deprivation of liberty.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties. 

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services. 

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire 
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings 
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers 
  • Numerous mental health and learning disabilities services

Qualifications and Knowledge

Essential criteria
  • RGN Level 1 with current NMC registration
  • Completion of Management/Leadership training
  • Evidence of continuing professional development
  • Undertaken Practice Supervisor/Practice Assessor/ Mentorship training
  • Awareness of NMC Codes of Practice
  • Working knowledge of Hywel Dda Policies Procedures and Guidelines
  • Experience of speciality
  • Able to relate theory to practice
  • Maintain their personal and organisational professional development
  • Provide specialist clinical advice within the multidisciplinary team
Desirable criteria
  • Leadership/Management modules
  • Nurse prescribing
  • Current knowledge of Topical issues in Health and Social Care
  • Knowledge of Clinical Governance

Experience

Essential criteria
  • Experience of management of teams and clinical areas
  • Experience and understanding of managing staff including HR processes such as sickness and absence, PADR, recruitment/ interviews
  • Evidence of substantial community experience in a relevant setting
  • Able to demonstrate sound knowledge of relevant speciality
  • Statutory and mandatory training
