Job overview
**This position is open to staff working at the Trust in the ‘ECT Family’ this includes current East Cheshire NHS Trust employees, both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups.**
An exciting opportunity has arisen for a Band 6 Community Caseload Manager, to join our innovative Community District Nursing Team, supporting the communities of Bollington, Disley, and Poynton.
The Community Nursing teams of East Cheshire are leading the way in integrated working and best practice for the community nursing profession. Closer working relationships with colleagues in Therapies and Social Services have been formed, as part of working aligned to Care Communities with new programmes and projects being developed with other specialties, supporting multi-professional working.
Being part of our friendly, supportive and innovative team is an opportunity to take forward the expanding role of community nursing to meet the long-term condition agenda and the needs of our elderly population. Our dynamic management team are supportive and open to ideas and innovation.
Hours per week: 37.5

Main duties of the job
To provide holistic, high quality, safe and skilled health and social care / support to patients, carers and their families.
To work closely with member of the integrated care team to provide rapid and planned expert professional assessment and treatment interventions for patients in their own homes or other community settings, putting patients at the centre of their care and enabling patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible.
To be responsible / manage individual and team caseload of patients and carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of care programmes.
To provide leadership and support across the care community team demonstrating emotional intelligence to recognise pressures of self and team.

Qualifications
Essential criteria
- Current NMC (Nursing and Midwifery Council) Registration Registered Nurse (RN)
- Community Nurse Prescribing (V150)
- Mentor, Preceptor or equivalent
- Post registration qualifications e.g. palliative care, care of the elderly, wound care, continence or other relevant qualification.
Desirable criteria
- Specialist Nursing Practice (District Nursing) Degree or equivalent experience (and be willing to undertake/complete training within 2 years of commencing in post)
- Management/leadership Qualification
- Working towards MSc (Master of Science) in relevant subject.
Knowledge and Skills
Essential criteria
- Demonstrable experience at band 5 with either community experience or transferable skills.
- Achieved band 5 competencies required for a community post e.g. wound care
- Experience of mentoring junior, newly qualified staff and carers
- Advocate of multidisciplinary team working
- Knowledge and understanding of adult protection issues and safeguarding vulnerable adults and think family
- Recognition of health & well being agenda including health promotion and admission avoidance.
- Awareness of evidence based practice and NICE guidance.
Desirable criteria
- Demonstrable experience at band 6 with either community experience or transferable skills.
- Evidence of leadership skills
- Experience of undertake Continuing Health Care (CHC) assessments
- Advance communication skills
- Advanced care planning and health coaching
- Ability to forward plan and meet service needs i.e. appropriate scheduling and management of staffing issues.
Experience

- Rapid decision making and problem solving.
- Ability to lead and manage the team
- Ability to organise the workload, able to delegate and prioritise
- Good knowledge of health and safety and risk management
- Computer literate
- Effective written and verbal skills
- Relevant experience in assessing and managing of long-term conditions, palliative care conditions and wound care.

- Provision of support and direction to learner nurses, Health Care Support Workers, Nurse associates and new starters

Specific Job Requirement
Essential criteria
- Must be able to undertake all aspects of the role whilst working as a lone worker
- Able to organise own workload and deal with unpredictable nature of the role
- Ability to travel across the county with heavy / bulky equipment and the ability to travel from patient to patient, whilst maintaining a consistent caseload
