The Children In Care team are looking to recruit one experienced nurse, who is enthusiastic and hardworking with experience of working with children in care.
We are looking for someone who has an interest in working with young people 16 year and over, who can also work flexibly, between 8:30 am and 6 pm, and fit around the children and young people.
The post holder will spend time both within an office environment and travelling between service users and other services within Cheshire West and Chester footprint. Access to transport is essential for this role.
The successful candidate will be a nurse who has a degree in specialist community public health nursing or qualification recognised as equivalent by professional body and with the appropriate registrations.
Main duties of the job

You will be lone working visiting children and young people in a setting of their choosing – at home, in school, face to face or virtually. You will be required to carry out an holistic health assessment and make any referrals or recommendations suited to that young persons needs.
You will have excellent communication skills with an ability to work in multi-disciplinary environments.
You will demonstrate an ability to work autonomously but will receive appropriate levels of supervision within the team.
You will liaise closely with Cheshire West and Chester's Supporting Independence Team, where you will dedicate 1 day per week to work alongside.
