Job overview
This is your opportunity to join a forward-looking community team, if you have previous experience in this area of nursing or if you are new to this specialty.
The Community Nursing Team works as part of a wider multi-disciplinary team, caring for residents in Congleton and Holmes Chapel. We work in partnership with practice nurses and empower patients to deliver the lower leg wound pathway. We enable patients to manage their long-term conditions whilst remaining at home and preventing unnecessary admissions to hospital. Our work enables us to integrate with other services in the community including virtual wards and crisis response.
Congleton War Memorial Hospital is currently undergoing exciting changes and opportunities to link with the development of the community diagnostic centre.
Hours per week: 30 hours

Main duties of the job
The successful candidate will have excellent, up to date clinical skills & be enthusiastic, motivated and be able to work flexibly across our patch. It is expected that candidates will be able to contribute to the on-going advancement of the team & will be rewarded with a friendly, supportive environment in which to develop.
The nursing team is part of a wider care community in which we support Congleton and Holmes Chapel residents to live well
Experience is not necessary & enquiries are welcome from those interested. Informal visits are encouraged, especially from those currently working in the acute sector, who would like to see how community care works, first hand.

Qualifications
Essential criteria
- Registered Nurse to degree/diploma level
- Current registration with NMC (Nursing and Midwifery Council)
- Evidence of recent professional development in an up-to-date portfolio
Desirable criteria
- Non-medical prescriber
- Teaching/Assessing qualification
Knowledge and Skills
Essential criteria
- Ability to assess clinical situations and act appropriately
- Able to assess, plan and evaluate patient care, taking into account the needs and desires of the patient
- Awareness of effects of ill health and the ageing process
- Awareness of the principles and practice of SSKIN (Surface, Skin inspection, Keep moving, Incontinence, Nutrition)
- Computer literacy including Knowledge of Microsoft office systems and use of email
Desirable criteria
- Leg ulcer assessment and management
- Experience in palliative care, end of life care.
- Understanding of Gold Standard Framework
- Diabetes care/management
- Understanding and / or experience of managing patients with LTC (Long Term Care) needs
Experience
Essential criteria
- Recent previous experience within a comparable role/transferable skills
- Demonstrable knowledge of assessment and therapeutic interventions in area of practice
- Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
- Ability to prioritise and organise workload effectively
- Experience of using electronic patient/service user record systems
- Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
- Compassionate – with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic
