Job overview

We are seeking a motivated and compassionate Health Promotion Health Care Assistant to join our Bladder & Bowel team. This is a new role, where you will support health promotion activities aimed at improving the wellbeing of our community. You will work closely with Bladder & Bowel specialist nurses and physiotherapists  to deliver engaging health education and support services that encourage healthy lifestyles.

Main duties of the job

Nursing and administration duties may include

  • Assist the Special Bladder and Bowel Nurse/Support Nurse in the delivery of education e.g. study days, staff education, patients/carers.
  • Participate in health promotion activities.
  • Provide dietary and fluid advice.
  • Containment products review within the care home settings.
  • To support patients, families, and carers to achieve their optimum health and independence.
  • Promote fundamentals of bladder and bowel care and prevention of risk associated to bladder and bowel dysfunction using evidence-based practice.
  • To support the Specialist Adult Bladder and Bowel Nurses, Physiotherapists, Clinical Lead and administration team in the day-to-day operational functioning of the service.
  • Participate with other multi-disciplinary / multi-agency staff who may require gaining an overview of the Support Assistant role and assist with the induction of new staff into the team.
  • Act as a role model for excellence in the delivery of bladder and bowel care and the MCHFT Trust ‘Values and Behaviours'.
  • Work within and promote CCICP/MCHFT policy and procedures.

Detailed job description and main responsibilities

 

The post holder will work mainly under direct supervision from the clinical lead and SPN’s.  The daily workload will be varied with a range of delegated nursing activities / administration duties but expected:-

  • To work according to the MCHFT Trust’ Value and Behaviours’, supporting members of the Bladder and Bowel team to achieve the service specific and organisational objectives.
  • To provide a continual pathway of support for patients cared for within Central Cheshire Integrated Care Partnership (CCICP) Community.
  • To support prevention of harm to patients with bladder and bowel dysfunction and reduced risks associated with these conditions.
  • To increase quality of life for the patients and reduced stigma related to Bladder and Bowel dysfunction.
  • To provide advice and support to patients and care home staff junior in the delivery of high-quality care and actively support the Community Adult SPN and Clinical Team Lead in the delivery of training and care within CCICP and Care homes in the CCICP footprint.

  • Experience in working in community for the NHS

Qualifications

Essential criteria
  • 5 GCSE A-C qualifications or equivalent in English and Maths or equivalent
  • Proficient in microsoft applications including Outlook, Word, Excel and PowerPoint
Desirable criteria
  • Knowledge of medical terminology
