Job overview

This is  an exciting opportunity for a  Care Co-ordinator to work across the neighbourhood  community services covering the West Cheshire footprint

You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside team and partners including GPs and the voluntary sector, to support patients within the care community.

This post is a  Permanent, full time 37.5 hours per week position, Monday To Friday 9-5pm and requires travel between CWP bases.

Main duties of the job

This role will include the following -

  • Good interpersonal and communication skills.
  • Co-ordination of multi disciplinary meetings.
  • Management of of daily meetings.
  • Liaising with multi-disciplinary team members.
  • Signposting team members and patients/carers to relevant services.

Qualifications

Essential criteria
  • Good general education
  • Able to demonstrate a good command of English language, verbal and written.
  • Knowledge of Office Administration/Customer services acquired through training or experience to NVQ level or equivalent qualification.

Knowledge and Expertise

Essential criteria
  • Knowledge of Health and Social Care administration and processes.
  • Knowledge of Microsoft office applications, internet and email.
Desirable criteria
  • Knowledge/familiarity with medical terminology.

Experience

Essential criteria
  • Able to work well in a team
  • Experience/knowledge of issues of working with confidential information.
  • Experience of working with healthcare professionals/previous experience in the NHS.
  • Experience of working in an Office Environment
