Job overview

We are seeking to recruit a dynamic and enthusiastic Community Nurse to support in the urgent community two-hour response across Cheshire West. If you have experience of community working, enjoy a challenge and thinking outside the box, then this could be the post for you. 

We are looking for a highly motivated, well organised and flexible Community Nurse to provide assessment and treatment as part of a collaborative approach.  

As a Community Nurse the successful candidate will undertake duties involving the delivery of nursing services and the holistic assessments of patients' health needs using a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.

Main duties of the job

The successful post holder will work within our Hospital at Home Service to support patients to remain at home during episodes of acute health deterioration and reduced functional ability, with the aim of preventing unnecessary attendance at A&E, providing a response within 2 hours.
