Job overview

An exciting and unique opportunity has arisen to be a part of the Community Learning Disability Team, Intensive Support Service based in Leasowe, on the Wirral.
 
This service provides specialist assessment and intervention to help people with learning disabilities stay well and out of hospital.
 
As an Assistant Practitioner you will be expected to support the Multi Disciplinary Team in providing intensive support to a small cohort of people with learning disabilities, providing high quality levels of care in order to assist these people to live fulfilled and independent lives in a community setting.
 
You will be working within a forward thinking dynamic and supportive Multi-Disciplinary Team.
 
 

Main duties of the job

The post holder will be expected to work within a multi professional 
framework, managing a defined case load of adult service users with 
Learning Disabilities and complex health needs, within the boundaries set out by the registered practitioner responsible for the service users care delivery.
The post holder will be part of a team providing assessment and treatment to service users who may be in crisis, in a range of settings, and in partnership with other agencies, carers/family members and the wider multi- disciplinary team.
The post holder will be expected to deliver training to small professional
groups, service users and carers.
The post holder will be expected to work flexibly in varying locations and at 
varying times to meet the needs of the service

Qualifications

Essential criteria
  • Foundation Degree in Health care
  • Demonstrate commitment to ongoing training and development
Desirable criteria
  • A1 Assessors award
  • Up to date basic life support training
  • ECDL

Knowledge and Expertise

Essential criteria
  • Clinical skills and competency relevant to the role
  • Ability to Support clients in crisis.
  • Knowledge and management of clients with LD and other associated health needs
  • Record Keeping & report writing skills, including IT skills to manage electronic patient notes
  • Ability to Deliver training to other professionals, service users and carers
  • Basic working knowledge of legislation and policy relating to learning disabilities (e.g., mental health act; mental capacity act; Dolls, Mansell reports etc
Desirable criteria
  • Special interests related to one or more of the team’s priority areas (challenging behaviour; mental health PIMD or forensics)

Experience

Essential criteria
  • Minimum of 1-year Direct experience of working with clients with learning disabilities
  • Experience of personcentred planning and person-centred approaches to care
  • Experience of working with people with learning disabilities in a range of settings
Desirable criteria
  • Working within a community team or other MDT
  • Experience of working with people with a range of complex needs (challenging behaviour; mental health forensics; physical health)
  • Experience of co facilitating groups
